Summary:

The Admissions Registered Nurse is responsible and accountable for the conducting the initial assessment of all eligible residents and providing information about the initial examination and the course of action for the diagnosis with their treatment care team. Will also provide care of a group of residents: exercises autonomy by independent decision making related to resident care issues and problems; utilizes the nursing process by assessing, planning, implementing and evaluating care in accordance with established standards; collaborates with and may direct other health care team members in providing total care to assigned residents.

Essential Duties and Responsibilities:

1. Commits to the Vincentian's (VCS) mission to nurture and sustain a ministry of compassionate care that preserves the human dignity of persons within a diverse and changing society.

2. Upholds and promotes the VCS values of spirituality, compassion, quality, dignity, collaboration, advocacy and stewardship.

3. Promotes and supports a resident centered care environment that ensures the treatment of all residents, family members, visitors, fellow employees and customers with kindness, respect, and dignity.

4. Welcomes all new residents and coordinates all aspects of clinical care and physician orders.

5. Performs in-dept, systematic, pre-admission resident assessment and communicate pertinent information to appropriate Interdisciplinary Team (IDT)

6. Complete Admission required assessments per policy (i.e. Braden, Falls, etc.)

7. Initiate and complete Baseline Care Plan and other care plans as needed upon admission.

8. Complete Immunization and Polst.

9. Initiate orders and care plan for special devises or specialty instructions (daily Weights)

10. Add in Special Instructions the areas of focus for daily skilled charting. Adheres to all facility and department work rules, policies and procedures.

11. Evaluates resident’s needs, condition and care, and assists in developing nursing care plans for individual residents, including rehabilitative and restorative activities and instruction in self-help.

12. Welcomes new residents and coordinates aspects of clinical care/physician orders.

13. Admits and orients new residents.

14. RN Admissions Checks physician orders, transfer and pre-admission information. Reviews the resident’s medical history and social history.

15. Requisitions and arranges for diagnostic and therapeutic services as ordered by the physician within or out of the facility. Orders prescribed medications, requisitions supplies and equipment.

16. Performs supervisory duties as required.

17. Forwards diet orders to dietary department.

18. RN Admissions Transcribes physician’s orders to resident’s Electronic Medical Record (EMR).

19. Periodically reviews care plans with resident’s physician and nursing team.

20. Maintains narcotic records, unit records, and performs charting duties.

21. RN Admissions Orients and supervises training of new nursing employees.

22. Provides instruction and supervises assigned employees.

23. Assists other nursing staff in providing care to residents.

24. Performs and/or supervises individualized personal care for residents in accordance with the established nursing care plan and within scope of licensure.

25. Collects data, reports and records assessments for residents’ condition.

26. RN Admissions Assists physicians with procedures and examinations.

27. Participates in resident care conferences.

28. Makes referrals for spiritual and psychological needs to appropriate personnel.

29. Performs, assigns or supervises service duties and specific assignments such as cleaning instruments and equipment, collecting specimens, cleaning work areas and replacing supplies.

30. Attends required continuing education programs and unit staff meetings.

31. Maintains confidentiality of resident and staff information.

32. Anticipates and takes action to maintain a safe environment. Incorporates safety/risk management standards into daily work practices.

33. Performs all other duties assigned by supervisor.

Education/Experience:

1. Current licensure to practice as a Registered Nurse in the Commonwealth of Pennsylvania in good standing or a Temporary Practice Permit required.

2. Current CPR certification preferred.

3. Appropriate clinical knowledge of nursing care, significant signs and symptoms of disease, sterile and aseptic techniques, nursing procedures and treatment, and developed skills to provide technical nursing care required.

4. Previous experience as a Registered Nurse in long term care preferred.

5. Proficiency in Microsoft office, computer literacy, computer keyboarding skills, knowledge of basic office equipment and electronic health record knowledge.
