REPORTS TO: Director of Assisted Living

Position Summary

• The Assisted Living Health Services Coordinator is the nurse leader for Village Shalom’s assisted living. The AL Health Services Coordinator applies comprehensive knowledge of nursing practices and procedures for assisted living.
• Develops and updates Negotiated Services Agreements and Health Services Plans for assisted living residents.
• Conducts daily clinical rounds and audits. Coaches charge nurses and house supervisors on standards of practice for Assisted Living nursing documentation.
• Performs quality improvement responsibilities through assessment, improvement and evaluation of service delivery.
• Documents and reviews medication and treatment sheets for accuracy and compliance with physician orders, professional standards, federal and state regulations and company policy.
• Documents information regarding nursing care, care plans, and observations of the residents’ overall condition and behavior.
• Attends training to stay current with standards of care.

Education & Experience

• Must be a graduate of an accredited and certified school of Nursing.
• Must hold at a minimum a LPN or RN license in the State of Kansas or compact state. RN preferred.

Job Type: Full-time

Schedule:

• 8 hour shift
• Monday to Friday
• On call
• Weekends as needed
