Manager of Resident Experience

VillageCare 46&10 Assisted Living Facility is looking for a motivated RN to act as Resident Experience Case Manager. This role will implement the vision and strategy for an exceptional resident and family experience at our facility. Acting as the subject matter expert for case management and service excellence, while maintaining an active understanding of current thinking and innovative programs regarding coordination of service needs, will be essential to this role.
Some responsibilities will include:

• Greet and orient every new resident to ensure a smooth transition/admission to the facility.
• Evaluate at the time of admission/readmission and periodically, at least once every 6 months, the needs of a resident and the capability of the program to meet those needs.
• Identify needs and arrange for services and entitlements from public and private sources for income, health, mental health, and social services.
• Appropriately document all Case Management activities performed on behalf of residents to ensure compliance with New York State Department of Health documentation regulations.
• Monitor all complaints and grievances and ensure they are resolved in a thoughtful and patient centered manner.
• Monitor, report, and analyze data to ensure responsiveness and alignment throughout the facility.
• Actively participate in weekly Management Meetings to facilitate the coordination of overall program operations.
• Develop service and educational programming throughout the facility relating to the resident experience.
• Oversee the resident satisfaction survey process, assesses the results of the survey, and implements programs/changes to improve experience.

We would like to speak with those who have a Bachelor's degree and Active NYS RN license. 5 years of related experience required.
