The SSVF Shallow Subsidy Case Manager is responsible for all duties associated with providing long-term shallow subsidy assistance to SSVF program participants and providing light touch case management services to participants receiving Shallow Subsidy rent payments. The Shallow Subsidy Case Manager will work collaboratively with the SSVF team to identify appropriate Shallow Subsidy participants and ensure accuracy and completeness of information provided by SSVF Case Managers to refer participants into Shallow Subsidy programming.

Compensation Range:
The base pay range is $23.00 to $25.00 per hour.

Essential Duties and Responsibilities:
• Review and accept internal referrals from SSVF Case Management team for Shallow Subsidy programming
• Provide light-touch case management services to Shallow Subsidy participants while working toward eventual self-sufficiency 
• Conduct monthly check-ins with Shallow Subsidy participants and collect income and other updated documents as needed
• Act as point of contact for landlords of, and advocate for, Shallow Subsidy participants in order to help maintain permanent housing
• Promote compliance with SSVF Shallow Subsidy participant guidelines
• Regularly coordinate and communicate Shallow Subsidy services with SSVF Case Managers and SSVF Housing Specialists
• Maintain accurate documentation of all Shallow Subsidy participants, and input services and expenses into the Homeless Management Information System.
• Maintain documentation of trainings, client contacts and other pertinent information in accordance with agency and best standard practices.
• Attend SSVF team and VVSD agency meetings as required.
• Other duties as assigned by supervisor.

Qualifications


Qualifications:
• High School Diploma
• Ability to rapidly develop rapport with SSVF program participants in order to build working relationships with client population.
• Must also be able to maintain client documentation. 
• Must be able to hold to high standards of confidentiality.
• Knowledge of HMIS a plus.
• Prior experience working with homeless population a plus.
• Ability to prioritize competing responsibilities and thrive in a challenging and frequently changing situations.
• Proficient skill level in Microsoft Office (Word, Excel, Power Point, Teams, Sharepoint)
• Valid California driver’s license and up to date vehicle insurance, as well as a clean driving record.
• Must be able to fluently read, write and communicate in English.
• Must be able to sit for long periods of time.
• Must be willing to use personal vehicle to perform work duties when necessary.
• Must be able to comfortably perform, both physically and mentally, all aspects of job description and other duties as assigned by supervisor.
• Experience supporting programs serving homeless individuals/families and those diagnosed with physical or mental disabilities a plus.
• Must be able to pass background check and drug screen.

Physical Requirements:
• Ability to bend and lift light objects
• Ability to sit for extended periods of time while viewing a computer monitor
• Dexterity of hands and fingers to operate a computer keyboard
• Capable of hearing and speaking to exchange information
• Ability to read reports and data
• Must be able to work in a noisy, busy environment.
