VenVisor Health, LLC is looking for a full time Medical Manager/Medical Concierge to join our team!

Job Requirements:
Must have previous experience in healthcare. Looking for a Medical Assistant, LPN or RN to assist our NP's remotely and in person for implementation of our Remote Clinical Programs (Chronic Care Management, Remote Patient Monitoring, and AI risk scoring).

The candidate will need to coordinate with technical, clinical and operational teams from different companies to set up care delivery systems.

Previous experience in Nursing Homes/Skilled Care facilities a plus. EHR experience needed. Point Click care experience preferred.
Must be able to communicate effectively with our Nurse Practitioners and work cooperatively throughout the day.
Must have the ability to work well with others in a TEAM dynamic.

Attention to detail required.

This is a hybrid position, with some travel required.

Job Duties:
Assist Nurse Practitioner with coordinating service of Remote Patient Monitoring, and Chronic Care Management. Responsibilities will include obtaining patient consents, coordinating screening of residents and reviewing patient information. Will assist NP with coordinating services, including review of Electronic Health Record. Maintain HIPPA compliance. Communicate and interact with facility staff. Communicate daily with NP's, report to Clinical Director.

Skills:

- Excellent communication skills, both verbal and written
- Strong customer service orientation
- Ability to handle phone systems and navigate through various software applications
- Attention to detail and ability to multitask in a fast-paced environment
- Knowledge of medical terminology and healthcare processes is a plus
