Schedule: Sunday 3p-11p, Monday 3p-11p, Wednesday 3p-11p, Thursday 3p-11p
Total hours: 32
Pay Rate: $31.00 per hour
The Residential Licensed Practical Nurse (LPN) reports to the Residential House Manager (RHM) for all programmatic duties, and is supported by the Director of Nursing for all nursing-related duties, policy, and practice. The LPN will attend person-supported medical appointments whenever possible, provide nursing care during on-duty hours, follow plans of care for each person in the residence and facilitate health education and training of the direct care professionals in order to ensure the health and safety of the people served. This position will be compensated on an hourly basis.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
• Assessments specific to each person’s health care needs.
• Assist in creating Problem/Needs list for each person in the residence with the nursing administrative team
• Review nursing Problem/Needs list bi-monthly, after each hospitalization or as needed.
• Administration of medications as assigned.
• Assist with Health Care Provider visits.
• Write daily shift progress notes and weekly nursing assessments.
• Notify the Health Care Provider and Director of Nursing of any medical changes or concerns then communicate and document all information.
• Communicate any discharge instructions, treatments or medication changes to the RHM and all staff.
• Work with Home Health nursing if applicable.
• Physically assist individuals in transfer and carries, both ambulatory and non-ambulatory.
• Physically assist persons in bathing and personal hygiene tasks which may include lifts and physical support to persons with body weights of 150 - 250 pounds.
• Educate and assist staff and individuals with meal preparation, nutrition, and dietary needs.
• Follow agency policies and procedures as defined in the agency’s Policy and Procedure Manual.
• Identify safety risks and possible alternatives and advocate for the rights of all individuals.
• Maintain confidentiality in keeping with HIPPA regulations.
• Handle related job duties in compliance with agency’s Programmatic Policy Manual.
• Other duties as assigned.
MARGINAL DUTIES AND RESPONSIBILITIES:
• Ensure the program maintains the standards outlined in the Department of Developmental Services mission statement, the Survey and Certification Tool, as well as requirements of Performance Based Objectives.
• Attend and/or participate in training of the residential staff during their staff meetings.
• Attend nursing team meetings.
• Implement the agency mission through service delivery, staff development, and resource acquisition.
• Provide recommendations to your supervisor and Director of Nursing regarding ways to improve the current systems
• Observe all Agency policies and procedures
• Accept responsibility and accountability as part of the position
• Maintain confidentiality and positive demeanor while successfully managing multiple priorities simultaneously.
• Maintain CPR certification.
• Maintain Safety Care certification, if required by position/program.

• Complete MAP training audit
• Handle all projects with a positive attitude
• Ability to establish rapport with the disabled population, their families, co-workers and service providers.
• Organized, dependable and even temperament; good phone manner.
• Effective verbal and written communication skills
• Strong interpersonal and communication skills, and the ability to work with a wide range of employees.
• Must be able to bend, lift, stoop, reach and file.
