The Coordinator of Care Management will be responsible for directing assigned service line operations whose primary focus is case management, social work, utilization review, discharge and transitional care planning. Duties will include leading multidisciplinary teams, overseeing total care management in compliance with applicable standards and regulations, reviewing/revising policies and procedures, development and maintenance of a yearly budget, and encouraging collaborative relationships with an interdisciplinary team that includes the hospitalist, nursing, ancillary staff, home care, hospice and other departments/agencies that may be involved in care management of our patients..

The qualified candidate must have a Masters in Social Work along with broad clinical expertise; exceptional leadership skills; effective interpersonal and strong communication skills, organizational, analytical and problem-solving skills. He/she must be a strong team player with the ability to prioritize, manage time effectively and be flexible in a dynamic CAH work environment.

Experience
Eight (8) years of related healthcare experience, preferably in the hospital setting, to include five (5) years of experience in Utilization Management, Discharge Planning, Case Management and Care Coordination.
