What You Will Do:
The Case Manager is responsible for providing supportive clinical services to patients and their families as a part of the multidisciplinary team with most emphasis on supporting eligibility screenings, coordinating with community partners, coordinating/facilitating discharge plans and auditing the medical record. Case Managers are an essential partner in our mission to serve our community by improving health and well- being while collaborating with our community partners.
Responsibilities and Duties
Provides administrative support to the Outpatient Behavioral Health Program.
Coordinates the admission process.
Provides insurance re-certification and/or school and work forms as needed.
Functions as an information source for all inquirers.
Ensures required signatures have been obtained, assists with medical records and supports with data entry/tracking census information.
Collaborates and coordinates care with outside providers.
Responsible for crisis response to mental health and/or medical crises in conjunction with the treatment team to promote crisis stabilization.
Assists the Clinical Supervisor and/or Program Manager and members of the treatment team as needed.
What You Will Bring:
Education

Bachelor’s degree in Social Work, Psychology or related field required
QMHP/QMHP-Eligible
Experience
Minimum of two years relevant experience required
Certification & Licensures
BLS Certification (Basic Life Support) – American Heart ‘Healthcare Provider’ (HCP) – AHA approved required*
Certification in case management desirable.
* New hires must have American Heart Association (AHA) appropriate certification prior to completion of orientation.
Qualifications
Ability to work independently required.
Presents a positive image by professional appearance, attitude, personality, customer service and body language.
Ability to multitask, prioritize deadlines, and work well in both a team environment and independently required.
Excellent organizational skills required.
Excellent verbal/written communication and interpersonal skills required.
Knowledge of Microsoft Outlook and Word for data collection capabilities required.
Ability to build rapport with patients with a positive attitude and commitment to treat patients with dignity and respect required.
Ability to effectively assess, plan and implement therapeutic services in a multidisciplinary setting required.
Ability to mobilize and coordinate resources effectively required.
Knowledge of characteristics and service needs of persons with mental illness, and/or substance use history, behavioral disorders, language disorders, and other sensory impairments.
Knowledge of community mental health and public health in order to provide advocacy and enhance individual’s treatment and access to resources.
Knowledge of principles, procedures, techniques, trends and literature related to mental health, substance use, and psychiatric/medical medications/treatments.
Knowledge of specific local, state, and federal regulations and resource eligibility.
General knowledge of counseling and behavioral management skills with dually diagnosed persons.
Ability to respond effectively to changes in the work environment in a positive and professional manner.
