```Duties``` - Provide direct care and support to individuals with developmental disabilities, dementia, or hospice care needs. - Assist residents with activities of daily living, including bathing, dressing, grooming, and toileting. - Administer medications according to prescribed schedules and document medication administration accurately. - Monitor and record vital signs and report any significant changes or concerns to the appropriate healthcare professionals. - Implement and follow resident care plans as directed by the supervisor or case manager. - Ensure the safety and well-being of residents by maintaining a clean and organized living environment. - Promote resident rights and advocate for their needs and preferences. - Assist with meal planning, preparation, and feeding as necessary. - Provide emotional support and companionship to residents. - Collaborate with healthcare professionals, case managers, and other team members to ensure coordinated care. ```Requirements``` - High school diploma or equivalent. - Previous experience working with individuals with developmental disabilities, dementia, or hospice care is preferred. - Knowledge of medication administration protocols and procedures. - Ability to effectively manage conflicts and resolve issues in a calm and professional manner. - Familiarity with resident rights and regulations governing care facilities. - Strong organizational skills and attention to detail. - Ability to perform basic first aid procedures when necessary. - Excellent communication skills, both verbal and written. - Ability to work independently as well as part of a team.
