Buffalo Veteran's Hospital is looking to hire a Community Nurse Referral Coordinator .One full time variable day position, Monday through Friday, 9 hour shifts
ROLE AND RESPONSIBILITY:
The Community Nurse Coordinator is directly responsible to the Geriatrics and Extended Care (GEC) Care Line Manager. He/she will work in conjunction with other GEC disciplines, serving as a POC for Community Nurse Referral office for options available to Veterans who are eligible and choosing to use Extended care services in the community. The incumbent is responsible for planning, coordination, implementation, and evaluations of all aspects of GEC purchased care programs, utilizing various payor sources to include private insurances, Medicare/Medicaid, VA pay, CCN, VCA, and IDIQ. The coordinator will have contact with agencies/ facilities and Veterans/families regularly. Conduct any annual assessments and in some instances include site visits (Travel) depending on or coordinators responsibility to ensure quality care is being provided. As a core member of the Geriatrics purchased care Interdisciplinary team functioning as a resource for MDs, SWs, pharmacy, psychology, and students for GEC. Coordinator ensures All Purchased Long Term Services and Supports (PLTSS) community providers caring for Veterans at VA expense must follow applicable federal or state regulations. VA Medical Centers (VAMCs) may purchase care in the community under multiple programs and payment authorities, as outlined in the 10N Memorandum “Options for Providing Community Care (dated June 12, 2017). As aligned with Institutionalized/Noninstitutionalized Care (NIC) the community nurse referral coordinator accepts clinical/ administrative responsibility focusing GEC age on place programs within the department. Provide education on programs to all veterans/families, vendors, and clinicians, who are seeking referral for GEC services.
QUALIFICATIONS TO MAKE YOU ELIGIBLE FOR A POSITION:
1) Minimum of three years of clinical RN experience required
2) BSN required
3) Demonstrated ability to communicate effectively both orally and written required
4) Demonstrated ability to work independently with minimum supervision required
5) Demonstrated ability in areas of interpersonal relations, critical thinking, problem solving, and conflict resolution required.
6) Current and unrestricted drivers’ license required
7) Knowledge of CCN, HSRM, PPMS, and Microsoft Office package and demonstrated ability to use the program within it such as word and excel preferred
8) Knowledge of Medicaid/Medicare & VA benefits and eligibility preferred
9) RN geriatric experience with patients diagnosed with progressive memory loss preferred
10) MSN preferred
