VA is seeking a RN for their primary care team located in Buffalo, New York.
Staff Nurse: RN Care Manager Position; Patient Aligned Care Team (PACT2) for Primary Care
ROLE AND RESPONSIBILITY: The RN Care Manager uses the nursing process
and evidence-based practice to collaborate with the "CORE TEAM" (Patient
Primary Care Provider, RN Care Manager, LPNs, PCT/LPNs, and MSAs) and the
EXPANDED TEAM (Family/Caregiver, Internal and Community-Based services
involved in providing care to the patient) in developing and implementing
the patient-driven holistic care plan for life. The RN Care Manager
coordinates care, and provides direct patient care and patient and family
health education with a focus on self-management, prevention, and
wellness, based on the patient's goals. The RN Care Manager demonstrates
leadership by serving as a PACT leader, and serves as a preceptor or
mentor to colleagues or students to enhance professional growth,
development and practice.
QUALIFICATIONS :
1. .Demonstrated ability to function effectively without direct daily supervision.
3. Demonstrated ability to perform accurate physical assessments, triage patient needs, and make sound, independent clinical judgments resulting in appropriate plans of care and improved patient outcomes.
4. Ability to organize and prioritize own work.
5. Demonstrated ability to function effectively as a contributing member of a multidisciplinary team in a variety of settings.
6. Demonstrated ability to coordinate patient care across the continuum of care including but not limited to non-VA care, inpatient hospitalizations, specialty care, palliative care settings.
7. Demonstrated ability to interface with resources and programs in the community including co-managed care providers.
8. RN experience in either primary care, family practice, urgent medical care or acute medical/surgical setting.
9. Bachelor of Science Degree in Nursing preferred.
10. Demonstrated ability to lead a frontline healthcare team in process improvement, to meet organizational performance measures.

Pay: $80,000.00 - $117,000.00 per year
