Maternal Health Care Coordinator at Urban Baby Beginnings

Job Goal:

Urban Baby Beginnings seeks a dedicated Maternal Health Care Coordinator to work intensively with at-risk pregnant and parenting families, aiming to enhance maternal health outcomes, promote healthy pregnancies, and foster positive parent-infant attachments. This role emphasizes the importance of care coordination, community outreach, and direct service provision in the realm of maternal and infant health.

Location:

Richmond, Norfolk, and Petersburg, Virginia (In-person role)

Key Responsibilities

Maternal Health Advocacy & Support:

- Provide comprehensive maternal health support, emphasizing prenatal and postpartum care.

- Conduct home visits, hospital visits, and clinic-based consultations, ensuring timely and relevant care coordination.

- Offer evidence-based resources and information, particularly to families impacted by COVID-19.

- Facilitate Centering Pregnancy/Prenatal classes and support groups.

- Assist with birth planning, lactation support, and immunization education.

Community Engagement & Outreach:

- Collaborate with local community partners to promote UBB services and maternal health initiatives.

- Conduct neighborhood canvassing, street outreach, and presentations to increase awareness and engagement.

- Engage with key community influencers (e.g., barbers, coaches, pastors) to disseminate maternal health information.

- Organize and distribute resources, such as diapers and essential supplies, to families in need.

Care Coordination & Case Management:

- Manage an active caseload of pregnant and parenting families, ensuring consistent and quality care.

- Conduct regular check-ins via video, phone and email, maintaining open communication and support.

- Collaborate with healthcare providers, doulas, home visitors, and other stakeholders to coordinate care plans.

- Research and connect families to relevant community resources, services, and support systems.

Data Management & Compliance:

- Maintain accurate and timely documentation of client interactions, services provided, and outcomes achieved.

- Collaborate closely with program coordinators to ensure compliance with program standards and objectives.

- Monitor and report on program trends, successes, and areas for improvement.

Professional Development & Team Collaboration:

- Participate actively in training sessions, staff meetings, and professional development opportunities.

- Foster positive relationships with colleagues, clients, and community stakeholders.

- Uphold the mission, values, and policies of Urban Baby Beginnings in all professional endeavors.

- Other duties as assigned

Qualifications:

- Bachelor’s degree in public health; nursing, social work or relevant degree with experience

- Minimum of 2 years of relevant work experience, preferably in maternal and child health.

- Strong interpersonal, communication, and organizational skills.

- Ability to work collaboratively in a fast-paced, team-oriented environment.

- Proficiency with Microsoft Office applications and relevant technology platforms.

- Commitment to equity, anti-oppression, and reproductive justice principles.

Compensation & Benefits:

- Full-time position with up to 40 hours per week
