We are a 120 bed Skilled Nursing Facility located in Cortland, NY and are now accepting resume's for a full time Social Services Assistant. Under the direct supervision of the Director of Social Services, the Assistant will be responsible for ensuring that the overall psycho-social, emotional and social needs of the residents are met and maintained on an individual basis in accordance with Federal, State and Local standards. Following policies & procedures of the facility to ensure that our residents maximize independence and receive quality individualized care.

Job duties include:

• Coordinating and completing intake and discharge planning procedures for the facility.
• Completes depression scales, psycho-social assessments and counseling for residents and families.
• Ensures that all required paperwork such as MDS's, care plans, chart documentation, etc, is up to date.
• Communicates with residents, families and other departments on a regular basis to make recommendations as to the resident's abilities to function effectively at home and makes referrals to outside agencies as necessary.
• Attend meetings such as Care Planning, Quality Assurance and daily morning meetings to inform other members of the interdisciplinary team of resident updates, etc.

Requirements:

Minimum of a Bachelor's Degree in Social Work, Sociology or Psychology with 1 year experience in a Skilled Nursing Facility.
