Uplift Solutions is Hiring a Trauma Informed Specialist/Case Manager Near Philadelphia, PA

Trauma Informed Specialists provide comprehensive case management services for an adult reentry workforce development program. They directly support participants as they navigate their reentry and career pathway - working in partnership with Uplift team members and external partners to assess participant readiness for programming, identify strengths and challenges to achieving goals and remove barriers. Case Managers are a critical link between participants and the various elements of Uplift’s workforce development program.

Trauma Informed Specialists work closely with other program staff to assess and monitor participant progress. They take proactive steps to coordinate referral support that directly responds to participants’ needs. This position also works with external partners (such as literacy, behavioral health and housing providers) to ensure that services are coordinated and participants are able to fully engage in career pathway services. Case Managers are responsible for successfully guiding participants through benefits navigation, reentry/compliance with post-release reporting, and successful transition to full time employment.

Uplift utilizes two case management models: Strengths-Based Clinical Case Management Model and the Intensive Case Management Model. Case Managers design and maintain a clear sequence of tasks, decisions, and management approach throughout all phases of case management: intake, needs assessment, individual planning, service delivery and monitoring. Case Managers maintain client contact for a period of 3 years following program completion.

Trauma Informed Specialists provide data-informed, risk-informed case management that facilitates prioritization of resources. As Uplift transitions to a more robust case management system that can track highly individualized services and capture complex data and metrics, Case Managers will be responsible for ensuring timely, accurate and complete data entry.

Trauma Informed Specialists play a critical role in program evaluation. Trauma Informed Specialists collect and analyze data and information to inform future programming and grant proposals. They provide data and reports that facilitate grant development and reporting to funders. They also use this information to create prioritized lists of resource needs and inform s outreach and collaboration strategies.

Recruitment

· Attend weekly Parole orientations

· Conduct phone interviews; conduct in Person Interviews at halfway houses/recovery houses

· Conduct background checks for potential participants

· Phone calls to future participants once they are accepted and create profiles

Planning

· Follow up on initial recruitment; draft acceptance letters to Probation/Parole/halfway houses

· Coordinate and facilitate patient care through assessment, evaluation, planning, and implementation

· Conduct individual assessments; tool may vary based on program. Determine eligibility based on funder requirements in accordance with established criteria

· Provide referrals for alternative services when applicants cannot succeed in the available programms

· Administer needs assessments to determine services that would benefit program participants.

· Assess client’s current and potential strengths, abilities, and needs; determine how these impact client’s ability/inability to function autonomously.

Referrals/Outreach

· Conduct initial orientation and collect participant information

· Develop and maintain positive relationships with others, including training partners, program participants, probation/parole officers and other collaborating partner staff.

· Determine the need for and coordinate referrals for supportive services; conduct outreach and connection to community resources to address barriers to employment and life management challenges.

· Identify and refer clients to community resources including employment training and placement, housing assistance, mental health and substance use treatment, public benefits, identification obtainment, legal services, and more.

· Serve as a liaison between program participants, community organizations and family to ensure networks of support are effectively communicating.

· Assist in development and maintenance cooperative agreements, grants, contracts, etc

· Support the development of relationships and liaise with local, county, state, and federal governments, community groups and local businesses.

Direct Services

  • Provide case management services including but not limited to the creation of Individual Success Plans development; Facilitate multiple care aspects (case coordination, information sharing, etc)
  • Assist with the facilitation of support/counseling sessions - individual or group; Assess and address motivational and psychosocial issues

· Deliver workshops/curriculum which includes: psychoeducational classes; motivational sessions; professional development; goal setting; orientation to work-place expectations; communication skills; basic skills testing; basic computer use; basic workplace skills; job search skills; job applications, resume writing, interviewing, etc.

  • Act as the patient's advocate as it relates to insurance coverage and financial assistance
  • Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options

· Provide information and referral to community resources to help participants address their employment and life management barriers that could affect ability to achieve career goal.

o Housing support, Benefits navigation, Clothing assistance/setting appointments with Career Wardrobe, Communication with halfway houses, Connecting participants to outside therapy/counseling

· Provide case management for participants at specified intervals to assist in the continued prevention and elimination of barriers to successful employment/advancement.

· For program graduates, continue to provide information and referral access to community resources to help participants address identified barriers which may hinder successful employment.

· Proactively mange case assignments, conduct regular reviews case progress and apply

  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered

· Maintain accurate case notes/files for current and former participants

Coordination

· Work closely with trainer, career coaches and Staffing Specialist to communicate existing barriers and barrier resolution plans for each workshop participant as need.

· Coordinate with community partners as to prospective participant’s status and needs.

· Communicate with prison/jail personnel including parole/probation and court officials.

  • Arrange abeyances with social services, health and governmental agencies

· Drafting acceptance letters to Probation/Parole/halfway houses

  • Follow up with participants post-graduation (monthly for 1st 6 months, then biannually) and referral to relevant services as needed and tracking impacct

Data and Reporting

· Maintain accurate participant files and records according to program guidelines (HubSpot and/or other systems)

· Ensure participant profile includes demographic data (race/gender/education/ location)

· Ensure database include graduation rates and employment

· Ensure database includes retention data

· Ensures there is a one-stop-location for data and information

· Creates automated reports that can be accessed throughout the organization

· Enter all data in electronic case management system with 48 hours of service delivery

· Record and track data relative to an individual’s assessments.

· Apply standard criteria to determine when case closure is appropriate

· Tracking all relevant information related to participant contacts

Administration

· Submits weekly payment reports to CFO

· Takes all phone calls relating to unplanned absences when participants are sick, have emergencies, or will be late etc.

· OTHER

Other

· Adhere to professional standards as outlined by protocols, rules and regulations

· Perform all job functions in cooperation with the Director, Lead Case Advisor and other Workforce Staff

· Participate in staff meetings and other events as needed

· Attend and participate in regularly scheduled supervisions sessions

· Promote within the agency and with the general public the philosophy and practice of social rehabilitation

· Perform other related duties, responsibilities and special projects as assigned

· Cover Life Skills class when needed

· Office manager duties when needed

Qualifications for Case Manager

  • Bachelor’s (Masters Preferred) in Social Work, Psychology,
  • Certified Case Manager, preferred
  • 3-5 years of clinical experience, preferably in case management, or related experience
  • Strong written and verbal communication skills
  • Excellent clinical assessment and analytical skills
  • Previous experience working with adult community and reentry programming preferred.
  • Experience working with diverse populations, particularly reentry and young adults with economic disadvantages
  • Strong organizational, time management and problem solving skills
  • Solid team player
  • Experience with data entry and proficiency with computers including Microsoft Office Suite; Word, Excel, Power Point, Publisher, Outlook, and Internet, and electronic data base systems.
  • Demonstrated understanding of community resources and how to access them
  • Ability to work independently, be flexible and dependable, and demonstrate initiative and creativity

Job Type: Full-time

Pay: $50,000.00 - $55,000.00 per year
