unm is Hiring a Community Health Worker Near Albuquerque, NM

Works closely with medical providers, mobile medical teams,and social services agencies to provide medical care, resources, and educationto clients. Will perform a wide array of medical and clinical support functionsin a variety of settings (clinic, shelter, street, home). Will identify andbuild rapport with individuals experiencing homelessness who may be living onthe street or in vehicles. Will keep records and documentation of clientinteractions, support providers during procedures and examinations, and assistwith scheduling.

UNM HSC is the only academic medical center and is the primary teaching hospital in New Mexico. We provide care for over 200,000 New Mexicans and is the only Level I trauma center in the state. Our outpatient community clinics are Level III NCQA recognized Patient Centered Medical Homes and serve a culturally rich and diverse community. Clinical sites are integrated with clinical pharmacists, behavioral health, and care management teams.
