POSITION TITLE: Direct Support Professional

SUPERVISOR: Program Manager, Director of Community Services, Asst. Director of Community Services

GENERAL DESCRIPTION:

A Direct Support Professional (DSP) is a member of a designated service provision team with direct responsibility for the implementation of the Individual Plan (IP) and related support services designed for each client participating in the program. A DSP is primarily responsible for working with people with developmental disabilities in their homes and the community. Position requires flexibility in scheduling, use of good judgment, initiative, punctuality, good typing and writing skills, the ability to work autonomously and independently. A DSP is required to follow established agency policies and procedures to ensure general health, welfare and safety for all clients participating in the designated program. DSP's may provide support services in a variety of functional settings. Use of the DSP's personal vehicle may be required.

ESSENTIAL DUTIES INCLUDE BUT ARE NOT LIMITED TO:

Implementation and documentation of Individual Plan (IP) and/or Positive Behavior Support Plans under direct supervision of the Program Manager or Director. Review each Individual Plan upon implementation.

Provide services designated by the Individual Plan of each client. Services could include but are not limited to developmentally appropriate social/play activities, gross and fine motor activities, practical/independent living skills training and instruction, assisting with personal hygiene tasks, showering/bathing, transfers and lifts, domestic skills, meal preparation, and assisting with eating, health and safety issues, functional academics, financial management and community integration skills.

Supervise and provide for client participation in social, recreational, leisure and community inclusion activities at designated facility and in the community.

Provide safe and responsible transportation to and from routine activities, medical appointments, employment or social/recreational and community resource activities as required.

Respect clients' rights and respond to their needs. Empower individuals served to achieve their goals with dignity and respect.

Maintain appropriate sanitation, maintenance, care, use and security of facility or program property.

Administer or monitor self-administration of medications according to agency policies and procedures and the State of Missouri, as well as monitor general health and well-being of each client.

Responsible for maintaining client files including; accurate and thorough documentation of objective goals, support services provided, individual daily logs/data, communication logs, incident/injury/medication error reports or other program requirements.

Maintain home security of clients living in their homes. If necessary assist with client/family/guardian in recognizing and acting upon safety issues with in the home. Responsible for reporting circumstances that could adversely affect the health and safety of clients and their families to the Program Manager, Asst. Director or Director of Community Services and CCBSS Service Coordinator.

Participate in all required training or in-service programs as scheduled including CPR, First Aid, Level I Medication Administration, NCI, Abuse and Neglect, Positive Behavior Supports and Universal Precautions as made available.

Perform and complete other tasks and job duties as assigned by the Program Manager, Assistant Director or Director.

Communicate information necessary for the successful operation of the program to the Program Manager or Directors in a timely manner.

Promotes safety practices and procedures in all aspects of job duties, including, but not limited to: supervision of clients, program areas, medication administration, transportation, community integration, lifting, and equipment.

Abide by all CARF, Medicaid, Department of Mental Health, Health and Senior Services, HUD and Central Missouri Regional Office, Unlimited Opportunities, Inc. Policies and Procedures as a condition of employment.

QUALIFICATIONS:

Minimum:

Must be at least eighteen years of age.

Have a high school diploma or G.E. D. certificate.

Be able to lift/physically transfer clients as required by the needs of individual clients.

Be able to obtain a Missouri Class E License.

Must have completed 13 Courses of the College of Direct Supports.

Desired:

Prior experience working in community based services supporting persons with developmental disabilities.

Computer skills.
