This position partners with patients and families to ensure they understand follow-up needs and coordinates their medical appointments, as well as supports the families in a holistic approach and be a point of contact with complex needs.

Rare Disease Program Manager Overview

Key responsibilities include but are not limited to:

• Serves as a single point of contact for coordinating appointments, procedures, labs, imaging, or treatments
• Accurately navigates and translates complex service protocols, tools, standard work, and information to ensure appropriate services are delivered
• Gather and document information to help facilitate communication between the patient/caregiver and care team
• Maintains awareness of multiple competing situations needing attention and prioritizes to assure patient needs are met, while maintaining a pleasant, friendly demeanor.
• Identify potential issues that could complicate care (i.e. insurance issues, adherence concerns)
• Review referrals to the program and determine if patient meets criteria to be a part of the Rare Disease program
• Establish collaborative relationships with healthcare teams outside pediatric specialty and M Health Fairview

Minimum Qualifications:

• Bachelor's degree in Business or Healthcare
• 5 years of related experience
• Advanced experience with using Microsoft Excel, including pivot tables, formulas, and conditional formatting
• Intermediate experience with Microsoft PowerPoint, including charts, animations
