The Community Outreach Intervention Project is seeking a full-time Social Work Aide II (Medical Case Manager) for a Contract Appointment. This role will work under the general supervision of the Services Director, the individual will participate in developing a CM Quality Management Plan (QMP) that codifies the management and operational policies and procedures. The QMP will ensure work processes, products, or services satisfy expectations and quality standards of the Northeastern Illinois HIV/AIDS Case Management Cooperative. As part of the quality improvement duties, the incumbent will provide input in program development, implementation, monitoring, and evaluation of deliverables.

In addition, the Medical Case Manager will provide direct services to a caseload of 25-60 clients with high acuity ratings based on multiple diagnoses involving substance use, mental health, HIV status, and physical problems predating HIV. The case manager's goal is to help individuals living with HIV access primary medical care.

Duties & Responsibilities:

  • Determine client eligibility and enroll clients in case management. Develop initial service plans and link clients to appropriate social and medical services. Every six months, submit a reassessment and update service plans; revise plans when significant shifts occur in clients’ needs.
  • Assists clients with private/public health insurance through effective benefits management practices. Maintains and uses reminders in Provide and Outlook to track needed follow-up for client ADAP and other application submissions.
  • Receives and appropriately documents telephone and face-to-face meetings with program participants on an approximately monthly basis to assess needs. Work in conjunction with medical care providers (e.g., case conferencing, care conference), ensuring comprehensive medical care is provided and monitored based on client individual medical and psychosocial needs.
  • Maintains confidential client records, scanning and uploading documents using agency guidelines, including processing transferred records.
  • Performs chart audits to ensure compliance with program requirements and promote successful outcomes during site visits by funding agencies. Participate in monthly case conferencing with the UI Health Community Clinic Network (UCCN) medical team.
  • Receive, record, and transmit messages from program participants, communicate with community partners, respond to case management consults and referral requests to ensure the client's needs are being addressed.
  • The individual participates in required case management staff meetings and in-service training sessions. Trains new Medical Case Managers and provides backup assistance duties for other Medical Case Managers.
  • Travel to 5 field stations located in low socioeconomic areas of Chicago is required.

Minimum Qualifications Required:

  • One year of work experience comparable to a Social Work Aide I
  • Any combination totaling two years from the following categories:

A. work experience which provided knowledge of human behavior, including the ability to cope with a variety of people and/or understand the importance of people’s needs and feelings, in a public/customer service environment.

B. college course work in social or behavioral sciences or related field, measured by the following conversion table or its proportional equivalent:

  • 15 semester hours equal six months
  • 30 semester hours equal one year

Preferred Qualifications:

  • Through knowledge of services provided by various social service agencies.
  • Skill in interviewing patients/families to determine their physical, social and possible psychological needs.
  • Ability to obtain services necessary to meet patient/family needs.
  • Ability to explain and carry out facility and departmental policies and procedures.
  • Ability to work independently.
  • Ability to organize, prepare and maintain records, reports, files and correspondence.
  • Thorough knowledge of services provided by various social service agencies.
