Description:
The Community Outreach Intervention Projects seeks a Full-Time Social Work Aide II, Medical Case Manager to work at their storefront offices. The Medical Case Manager will provide direct services under AIDS Foundation Chicago's Co-operative Case Management system to community-based clients living with HIV/AIDS. The case manager will carry a caseload of 25-60 clients with high acuity ratings based on multiple diagnoses involving substance use, mental health, HIV status, and physical problems predating HIV. The case manager will provide client-centered, confidential services that link clients with a core, including legal assistance, medical transportation, emergency financial assistance, food, and housing. The aim is to help individuals living with HIV access primary medical care and medications, identify and remove barriers to medical care, and ensure adherence to the prescribed treatment plan to achieve viral suppression.
Duties & Responsibilities:
Minimum Qualifications Required:
1. One year of work experience comparable to a Social Work Aide I
2. Any combination totaling two years from the following categories:
A. work experience which provided knowledge of human behavior, including the ability to cope with a variety of people and/or understand the importance of people’s needs and feelings, in a public/customer service environment.
B. college course work in social or behavioral sciences or related field, measured by the following conversion table or its proportional equivalent:
