Description:

The Medical Social Associate (Outpatient Psychiatry Case Manager) provides case management services to clients receiving services in the outpatient psychiatric clinic. The case manager will work closely with clinic directors and clinicians to identify and address economic, social, and other structural barriers that prevent patients from receiving quality mental healthcare.
Duties & Responsibilities:

  • Working closely with department clinicians, provide case management services by performing client assessments, developing and implementing care plans, and providing transition/discharge services.
  • Perform assessments to identify barriers preventing clients from receiving quality mental healthcare, needs to address those barriers, and client resources.
  • Assist clients with referrals to community housing resources, food assistance programs, and utility bill assistance programs.
  • Assist clients in applying for job training programs/opportunities, unemployment and disability benefit programs, and referral to socialization programs.
  • Assist in referrals to government and community mental health services and resources.
  • Assist clients in obtaining and utilizing transportation benefits and services for clinic appointments.
  • Perform other related duties and participate in special projects as assigned.


Minimum Qualifications Required:

1. A. Bachelor's degree with a major in the social or behavioral science or related health specialty from an accredited college or university.

OR

B. Master's degree in social or behavioral science or related health specialty area from an accredited college or university.

2. One (1) year (12 months) of progressively more responsible full-time experience in the practice of social services/ agencies or medical/clinical setting maintaining acceptable standards of social work practices.
