Summary of Job Duties:The Program Coordinator will collaborate with an interdisciplinary team and Title V nurses under the leadership of the Family Centered Interdisciplinary Network (FIN) Director to provide a patient/family-centered model of care to improve access to health services. The Program Coordinator is trained in problem solving strategies and motivational interviewing to support engagement of the entire family in care.Qualifications:Bachelor's degree plus two (2) years of administrative program coordination experience or
Associates degree plus four (4) years of administrative or program coordination experience or
High School diploma/GED plus six (6) years of administrative or program coordination experience required.

Responsibilities:
Engage with families anywhere along the continuum, from the point of initial developmental screening to referral for initial developmental evaluation, diagnostic evaluation, and access to appropriate intervention(s).
Schedules monthly Family-Centered Interdisciplinary Network virtual meetings.
May perform other duties as assigned.
