Overview

Supervisor, Care Management

Full-time, Days

Responsible for supervising the day-to-day operations of the care management department, utilizing quality and cost-effective patient care principles including operations, staffing levels and employee relations. Collaborates with physicians and other health care workers, to assess, plan, implement and evaluate patient care. Serves as a mentor and resource providing leadership, support, and assistance to the care managers and social workers. Participates in shared leadership responsibilities that improve the quality of care provided to patients, enhances the work environment and the goals that outline our department. Serves an integral role in the multidisciplinary effort to identify and deliver quality and cost-efficient healthcare to patients. While the role is multifaceted, partners with the multidisciplinary team in guiding concentrated efforts to the oversight and collaborative coordination of care and services for a defined patient population, ensuring that adequate discharge plans are in place and completing utilization management and quality review activities.

Responsibilities

Leadership

  • Prioritizes, assigns and redistributes work, to include scheduling and delegating workflow of patient care in an organized manner.
  • Reviews/assigns patient assignments based on continuity and skill level of the team member.
  • Coordinates scheduling and maintains appropriate staffing levels and effectively communicates with others to ensure staffing requirements are met.
  • Collaborates with Manager to ensure unit-based orientation and training of newly hired team members is complete.
  • Collaborates with Clinical Educator to ensure the provision of staff education/in-services/specialized skilled training programs including competences. Collaborates with the Manager/Director, effectively resolving complex situations, such as patient complaints and crises as they occur within the department

Employee Development

  • Evaluates and guides staff through coaching and provides immediate feedback and responds to difficult inquiries and problems. Demonstrates the ability to interpret, apply and follow personnel policies/procedures fairly and consistently.
  • Provides feedback and input to Manager/Director for ongoing performance management.
  • Completes and issues performance evaluations with input from the Manager/Director.

Clinical Excellence

  • Possess a broad understanding of discharge planning strategies, level of care requirements, payments systems and associated requirements.
  • Upholds and role models the standards of excellence.
  • Supports and empowers staff to act with appropriate responsibility for the work processes and work environment of which they are a part. Evokes in staff a sense of ownership of and accountability for their work. As well as stimulate staff to continuously improve their performance.
  • Oversight, implementation, and evaluation of population-specific performance improvement initiatives, quality metrics, and core measures.
  • Models behaviors for demonstrating values in performing job responsibilities.
  • Provides leadership to improve performance, based on process improvement methodology and reports short- and long-term outcomes.
  • Provides specific and timely feedback to employees to recognize positive demonstration of values and correct performance that does not reflect values.
  • Provides oversight for utilization of evidence-based care for patient populations.
  • Assesses need for patient and family education. Participates with the healthcare team in identifying and providing health teaching and counseling, as appropriate. Facilitates referrals to appropriate resources.
Qualifications
  • Registered Nurse with bachelor’s degree or Social Worker with BSW. (MSW preferred)
  • Two years of nursing experience in acute care setting.
  • 2 or more years of nursing experience in acute care setting preferred.
  • Previous leadership experience preferred.
  • Registered Nurse licensed in Iowa if degree in nursing.
  • Ability to maintain current professional expertise.
  • Exhibits discretion and sound judgment in all aspects of the job.
  • Promotes positive interpersonal relations and serves as a role model within the department and with hospital personnel.
  • Writes, reads, comprehends and speaks fluent English.
  • Multicultural sensitivity.
  • Microsoft Office – basic computer skills.
  • Customer/patient focused.
  • Critical thinking skills using independent judgment in making decisions.
  • Demonstrates knowledge that patient and workplace safety is a priority by preventing errors, accidents and infections and reporting potential or actual concerns immediately.
