PRN home-based Nurse Practitioner
Delivers high quality care, experience, and improved economic outcomes through innovative home-based care services
Care at Home is a partnership between a patient’s primary care provider and other medical specialists
Hospital to Home provides hospital level care in the home to qualifying patients
Post discharge home visits focus on proactive and preventative health management and ED visit/readmission
Provide primary care, advance care planning, care management, annual wellness visits, and palliative care
Assess and manage a patient’s acute medical needs from the comfort of the patient’s home
Alternative option of care for qualifying adult patients who would otherwise be admitted to the hospital
Fully integrated system with clinics and hospitals on Epic EMR to improve care coordination
Responsibilities:
Seeing an average of 4 patients in the home daily
Participate in Call after hours and weekends
Provide Comprehensive assessment, intervention, education, preventative, and Urgent Care services for high-risk patients their home environment.
Provide alternate ambulatory-based healthcare to avert patients need to be seen in the ED, IP, or SNF
Work collaboratively as an extension of Primary care provider and Clinic Care coordination team
Assist patients with complex, acute on chronic, and serious illness with developing, implementing, communicating, and overseeing the patients’ home-based care plan
Collaborates and consults with the patient/family and health care team members
Treat non-life-threatening medical problems or conditions that could get worse if a patient were to wait to be seen at a clinic
Medical assessments and treatment planning
Home medical safety assessments
Medication review and supportive management
Lab draws associated with medical assessment and monitoring
Xray or ultrasounds associated with medical assessment and monitoring
Provide the best outcome for every patient every time
Qualifications:
Home Care, Hospice, Palliative Care, or Acute Care experience required
Must be willing to commute
Hold a current and unrestricted license or certification, as applicable in Advanced Practitioner’s field of practice in the state in which Nurse Practitioner provides services
Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioner’s field of practice
Employee maintains current licenses and/or certifications required for the position.

Excellent communication skills to effectively communicate with patients, physicians, and staff
