Job DescriptionDEPARTMENT: Community Services POSITION TITLE: Youth Services Coordinator FLSA: Non-Exempt
SUPERVISOR: Youth Empowerment Program Manager Job Code: NASALARY RANGE: $25- $28.75
OVERVIEW: As a Youth Services Coordinator (YSC), you will play a pivotal role in supporting the journey of youth towards self-sufficiency by coordinating and connecting them to a range of essential services. In this dynamic position, you will work and directly interface with participants and be responsible for managing and facilitating access to supportive services within the community, offering case management as required, and actively contributing to the development of programs and resources that enhance the overall well-being of youth.As with all UC staff, this position is responsible for adhering to the performance standards set forth in the UC Staff Employee and UC policies and procedures. Additionally, as with all staff, this position is a role model for participants in their development and attainment of goals.DIRECT REPORTS: N/A QUALIFICATIONS: REQUIRED:  
  • Bachelor's degree in social work, psychology, counseling, or a related field.
  • Proven experience in providing support services to transitional-aged youth.
  • Knowledge of social service programs, entitlements, and resources for at-risk youth, particularly at-risk or homeless.
  • Familiarity with issues related to homelessness and the challenges faced by transitional-aged individuals.
  • Strong interpersonal and communication skills with the ability to engage effectively with youth from diverse backgrounds.
  • Experience in case management and the ability to develop personalized service plans.
  • Understanding of trauma-informed care principles.
  • Ability to work collaboratively with other agencies and community partners.
  • Demonstrated organizational and time management skills.
  • Sensitivity to the unique needs and experiences of transitional-aged youth.
PREFERRED: 
  • Master's degree in social work, psychology, or a related field.
  • Previous experience working in a non-profit organization focused on homelessness prevention or support for transitional-aged youth.
  • Knowledge of guaranteed income programs and their impact on individuals transitioning into adulthood.
  • Training or certification in trauma-informed care.
  • Experience in program development and implementation.
  • Familiarity with local community resources and services for at-risk youth.
  • Ability to navigate and advocate within the social services and entitlement system.
  • Bilingual or multilingual skills relevant to the community served.
  • Commitment to the mission and values of the non-profit organization.
  • Ability to adapt to changing situations and a dynamic work environment.
As with all UC staff, this position is responsible for adhering to the performance standards set forth in the UC Staff Employee and UC policies and procedures.RESPONSIBILITIES INCLUDE (but not limited to): 
  1. Working directly with participants, Coordinate and connect youth to various support services aimed at fostering their journey towards self-sufficiency.
  2. Manage and provide access to essential supportive services within the community.
  3. Offer case management services based on individual needs and requests.
  4. Develop and implement programs/ workshops and resources that enhance the overall wellness of youth participants.
  5. Assist clients with applications for benefits and entitlement programs to ensure comprehensive support.
  6. Advocate for adequate, timely, and cost-effective services that align with the unique needs of at-risk youth.
  7. Respond promptly to any issues arising during service delivery, ensuring a seamless and supportive experience for participants.
  8. Assist clients in achieving and maintaining independent living in their home environments.
  9. Schedule appointments for clients to access necessary services and resources.
  10. Track and maintain accurate records of clients' service history and progress in HMIS database and other systems.
  11. Evaluate the quality of all services, identifying areas for improvement and implementing necessary enhancements using data to drive continuous improvement to help participants obtain their goals and to meet the outcomes of the contract.
  12. Collaborate with community partners and organizations to expand the network of available services.
  13. Foster positive relationships with external stakeholders to enhance program effectiveness and outreach.
  1. Organization Duties:
    1. Timely and accurate submission of timecards to supervisor
    2. Time away from the Office, Program and/or Home (i.e. vacations, personal time off, leaves of absence) must be pre-approved by supervisor and requested based on process outlined in the UC Employee Handbook.
    3. Maintain knowledge of staff handbook, policies, procedures, manuals, and pertinent documentation

  4. Resolve problems involving several concrete variables in standardized situations.
  5. Identify negative disruptive behaviors and redirect behavior using mediation and de-escalation techniques.
  6. Employees are occasionally required to stand; walk; sit; use hands to handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop; kneel, crouch or crawl; talk or hear; taste or smell.
  7. Employees must occasionally lift and/or move up to 25 pounds.
  8. Employees may be required to make or receive physical contact with clients and residents, such as in the process of delivering First Aid/CPR or de-escalation interventions.
  9. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
  10. Must have CPR and First Aid certificate within 10 days (about 1 and a half weeks) of the start date and maintain a current certificate with Human Resources.
  11. Must clear a Department of Justice background check including fingerprinting before start date and maintain clearance.
  12. Must be able to work in a DSS Community Care Licensed Facility.
  13. Must clear a health screening, including TB before start date and maintain clearance.
  14. Must have a car, valid California Driver License and be able to drive, have a clean driving record and maintain a clean driving record.
  15. Meet and maintain the minimum vehicle liability and property insurance limits (to be insured by our insurance carrier for using vehicle for business purposes such as transportation of residents or reimbursement of business-related mileage expenses).
