Unity Care Group is seeking an experienced DSP to care for individuals in the Hamilton County Area. Providing support to the individuals served including healthcare, nutrition and program implementing. Assisting Individuals with daily living skills and the development of independent community skills. This may include meal preparation, cleaning, organizing, taking out into the community for activities and participating in daily care of the home or individual. We have 2 individuals who live in the same home and have different needs. Client #1 is a male client who has lots of activities on the weekends and needs someone who is willing to engage with him on a continuous, active basis and willing to make sure he makes his activities and community outings. His schedule is schedule 1. Client 2 is a female who needs assistance. Her care is for Schedule 2 & Schedule 3.

Schedule 1:
Every Other Weekend

Saturday - 8AM-8PM
Sunday - 8AM-8PM

Schedule 2 -
Every Weekend

Saturday (into Sunday) - 11PM-8:30AM
Sunday (into Monday) - 11PM-8:30AM

Schedule 3 -
Every Other Weekend

Saturday 4PM-11PM
Sunday 4PM-11PM

Responsibilities may include:

• Assisting the House Manager in maintaining consistency in the provisions of supports and services.
• Accurately completing all on-site documentation; maintain current case notes and reports of Individuals progress and how it relates to their ISP.
• Coordinating and providing transportation for the individuals served.
• Ensuring cleanliness and upkeep of client homes.
• Ensuring the safety and wellbeing of the client.

Requirements for this position include:

• High school diploma or GED
• Valid driver's license
• Less than 6 points on driving record
• Must be able to pass a background check
• Must be able to pass a drug test
• Must be able to pass a physical
• 1 year Direct Service Professional (DSP) experience
