UCG is seeking an experienced Direct Service Professional (DSP) to care for an individual in a home setting that has disabilities. This position is part time, the expected time frame is 15-20 hours per week, with no more than 20 hours. The client is high function, mobile using a power wheelchair and very active. They participate in several clubs, organizations and activities. They're interested in someone who is a little tech savvy and willing to help them websites and social apps. The client has their own adaptive vehicle for transport for activties and appointments. Having someone who likes to be outdoors for walks, and light activities would be a plus. This role will include meal and snack prep to meet the client's high calorie nutritional needs. It will also include assisting the client with physical exercises and light house cleaning duties.

CPR/First Aid certification is required
DODD DSP Provider Training (mylearning.dodd.ohio.gov - Direct Support Professional (DSP) DSP 8-hour 2024) is required
Driver's License with less than 6 points on the driving record is required
Medication Administration - Level 1 is a PLUS.

Responsibilities may include:

• Providing support to the individuals served including healthcare, nutrition and program implementing.
• Assisting Individuals with daily living skills and the development of independent community skills.
• Assisting in maintaining consistency in the provisions of supports and services.
• Accurately completing all on-site documentation; maintain current case notes and reports of Individuals progress and how it relates to their ISP.
• Coordinating and providing transportation for community outings for the individuals served.
• Ensuring cleanliness and upkeep of client homes.

Requirements for this position include:

• At least 1 year experience in DODD field
• At least 18 years of age.
• High school diploma or GED.
• CPR/first aid (will train)
• DODD provider training (will train)
• Must be able to pass a basic background check.
