Wellness Coordinators will be providing needed information and resources to the ICT (Interdisciplinary Care Team), to drive Total Health Outcomes. Wellness Coordinators follow up on finding referrals, addressing escalations, connecting the teams, providing administrative support. 

Hours: Monday through Friday between the hours of 8:00am to 5:00 pm Eastern Standard Time

If you reside in Florida, you will enjoy the flexibility to telecommute* as you take on some tough challenges.

Primary Responsibilities:

  • Data collection from providers; input into multiple electronic documentation systems
  • Coordinate information to clinical team partners
  • Provide behavioral health benefit education
  • Collect discharge information from providers and document in relevant systems
  • Use Microsoft Outlook, Microsoft Teams, and other web applications to coordinate information with clinical team partners
  • Contacts the aftercare providers (individual practitioners and/or Community Mental Health Centers) and records aftercare appointments in electronic record system 
  • Assist the ICT (Interdisciplinary Care Team) in ensuring member is aware of his/her aftercare plan and assist with any additional behavioral health needs. Enrollees that do not have an aftercare appointment or missed their scheduled appointment are assisted in setting up a new appointment
  • Assists the ICT with knowledge of community support services
  • Assign cases as needed to ICT

Required Qualifications:

  • High School Diploma/GED (or higher) OR 2 years of equivalent Behavioral Health experience
  • 1 year of managed care experience
  • Intermediate level of proficiency with Microsoft Office (Outlook), Microsoft Teams, and other web applications
  • Access to a designated quiet workspace in your home (separated from non-workspace areas) and is able to secure Protected Health Information (PHI)
  • Must reside in the state of Florida
  • Ability to type 30 to 40 words per minute on a regular basis

Preferred Qualifications:

  • Bachelor's Degree (or higher) in mental/behavioral health field
  • 1 year of behavioral health setting experience (i.e. hospital, managed care organization, or joint behavioral outpatient practice)
  • Exposure to mental health and substance use disorder diagnoses and terminology
  • Healthcare experience
  • Casework experience at hospitals, social service agencies or community mental health centers
  • Medical/Behavioral setting experience (i.e. hospital, managed care organization, or joint medical/behavioral outpatient practice)
  • Prior work at home experience

  • Demonstrated effective organizational skills and scheduling skills as observed in day-to-day work
  • Excellent customer service skills
  • Excellent interpersonal and problem-solving skills
  • Attention to detail, as observed in day-to-day work

  • Must demonstrate flexibility, ability to follow through on all tasks in a timely fashion, good attention to detail and a willingness to learn

  • Able to handle sensitive issues with members and providers in a confidential manner
  • Demonstrates initiative in achieving individual, team, and organizational goals and objectives
  • Creative problem-solving skills with the ability to use community and network resources to meet members' needs
