MEMBER POSITION TITLE: Community Health Coordinator

OVERVIEW: The member will enhance community wellness efforts by supporting the Eat Well for Life Program. The member will assist in the operations of the Eat Well for Life program, including cohort planning, participant recruitment, data collection, and class facilitation.

Eat Well for Life is a 13-week Food as Medicine program that provides healthy food and education for individuals in the Fox Valley with Type II Diabetes. Participants receive healthy food boxes for themselves and their entire families, along with education on healthy living, food preparation, and diabetes self-management. The program is a partnership with Be Well Fox Valley, healthcare, hunger-relief, and other community partners, providing healthy food and education at no cost to participants; helping them learn to eat healthier and better manage their disease.

DESCRIPTION OF DUTIES AND RESPONSIBILITIES:

• Assist with identification of community members who qualify for Eat Well for Life (EWfL)
• Capture participation data throughout the program
• Coordinate and support EWfL health education sessions
• Support program operations such as preparing materials, food packing, transportation, and distribution to EWfL program participants
• Participate in weekly program review meetings to review progress, evaluation, and process improvement
• Perform 1 on 1 weekly check-in calls with EWfL participants
• Create marketing and communication materials for Be Well Fox Valley and participate in the promotion of the EWfL program (such as social media, create brochures, newsletters, etc.)
• Recruit and support volunteers necessary for the success of program strategies and activities
• Work with supervisor to deliver community-based programing around health and wellness
• Community outreach and programing around health and wellness topics and objectives
• Attend and participate in Be Well Fox Valley AmeriCorps Program trainings on a variety of topics, including program orientation all-member team training and networking opportunities, Public Health AmeriCorps CDC Train virtual training series, community service projects, etc. These trainings occur through a combination virtual and in-person.
• Complete AmeriCorps paperwork, such as monthly reports, feedback surveys, etc.

QUALIFICATIONS/SKILLS/EDUCATION/EXPERIENCE:

• Hold a high school diploma or equivalent. The ideal candidate would hold Bachelor's degree in Health and Wellness or related field. Examples include Community Health, Health Promotion and Wellness, Health Science, Nutrition, Dietetics. Background and experience based on host site need. Upper-level undergraduates with related experience are encouraged to apply.

• Fluent in Hmong and English
• Be 18 years or older at the beginning of their service
• U.S. Citizen, U.S. National, or Lawful permanent resident alien of the U.S.
• Must pass background check

OTHER INFORMATION:

• Member Term of Service: The Member’s service term will start on 6/3/2024 and end on 8/31/2024.There’s flexibility with the position start date, however the position must start by or before 6/10/2024.
• Time Requirements: The member is expected to serve 300 hours, which averages 24-28 hours/week during the service term period.
• Members will receive a gross living allowance while serving, which is paid on the 15th and last day of each month. The gross living allowance amount will be $676.47 per pay period (the amount listed is before tax deductions). Please note, there will not be an additional hourly wage or salary.
• Segal Education Award upon successful completion of hourly requirement described in Time Requirements: The Education Award amount for this position is $1,459.26. Please note, this education award is an electronic transfer to either your educational institution, federal loan provider, etc. Members will not receive the education award via direct payment (such as check, cash, etc.).
• Forbearance on qualified student loans while serving and interest accrual payments.
