United Way Fox Cities is Hiring a Community Connection Navigator AmeriCorps Member Near Menasha, WI

MEMBER POSITION TITLE: Community Connection Navigator

HOST SITE: United Way Fox Cities. 1455 Midway Road, Menasha, WI 54952

OVERVIEW: 2-1-1 is a simple, free way to connect all people in need with vital human services 24/7/365 for everyday life or in times of crisis. By dialing 2-1-1 people are linked to information about local health, human service and disaster related resources, 211 brings people and community resources together. There are 8 locally based 2-1-1 contact centers in Wisconsin covering all 72 counties, including the United Way Fox Cities (UWFC) 2-1-1.

The Community Connection Navigator will work collaboratively with UWFC and 2-1-1 staff, local agencies and other stakeholders to remove barriers and improve access to basic needs services.

DESCRIPTION OF DUTIES AND RESPONSIBILITIES:

  • Gain an in-depth understanding of local/statewide health and human service resources
  • Answer calls to 2-1-1 from community members seeking help with community non-profits and government services
  • Advocate for individuals experiencing difficulty or lacking the ability needed to navigate agencies, programs and services.
  • Facilitate, support, and coordinate collaborative initiatives including housing/shelter navigation, Falls Prevention, and Social Determinants of Health (SDOH). This could include attending community meetings, creating a project roadmap, helping design call or project workflows, calling individuals who need assistance in participating counties.
  • Attend outreach events and provide presentations on the 2-1-1 service.
  • Attend and participate in AmeriCorps program trainings on a variety of topics, including program orientation all-member team training and networking opportunities, Public Health AmeriCorps CDC Train virtual training series, community service projects, etc. These trainings occur through a combination virtual and in-person.
  • Complete AmeriCorps paperwork, such as monthly reports, feedback surveys, etc.
  • Community outreach around health and wellness topics and objectives
  • Deliver community based programing around health and wellness topics and objectives

QUALIFICATIONS/SKILLS/EDUCATION/EXPERIENCE:

  • Bilingual preferred – English/Spanish
  • Ability to communicate and work effectively and with a high level of professionalism with staff, community members, partners, and other stakeholders.
  • Self- motivated, goal oriented, and organized – ability to get the job done.
  • Able to work in unstructured environment and adapt easily to change. Takes initiative to complete assigned work.
  • Meets deadlines and completes tasks with a high degree of accuracy and dependability. Detail oriented.
  • Knowledge of Microsoft Teams and cloud-based technology
  • Passion for helping people.
  • Be 18 years or older at the beginning of their service
  • At minimum, hold a high school diploma or equivalent. The ideal candidate will hold an Associate or Bachelor’s degree in health and human services or public health degree or related background and experience. Upper-level undergraduates with related experience are encouraged to apply.
  • U.S. Citizen, U.S. National, or Lawful permanent resident alien of the U.S.
  • Provide information and complete, and pass a criminal history check

OTHER INFORMATION:

Member Term of Service: This position is a 15-month service term, which combines two consecutive AmeriCorps terms. The 15-month term will average 35-40 hours/week from 6/3/2024-8/31/2025. This position is eligible for health, dental, and vision insurance and place forbearance on qualified student loans during the AmeriCorps term.

Term one: The first term will serve 450 hours from 6/3/2024-8/31/2024.

  • The member will receive a gross living allowance while serving, which is paid on the 15th and last day of each month. The gross living allowance amount will be $1,014.71 per pay period (the amount listed is before tax deductions). Please note, there will not be an additional hourly wage or salary.
  • Receive a Segal Education Award upon successful completion of the term. The Education Award amount for this position is $1,824.07. Please note, this education award is an electronic transfer to either your educational institution, federal loan provider, etc. Members will not receive the education award via direct payment (such as check, cash, etc.).

Term two: The first term will serve 1,700 hours from 9/1/2024-8/31/2025.

  • The member will receive a gross living allowance while serving, which is paid on the 15th and last day of each month. The gross living allowance amount will be $958.33 per pay period (the amount listed is before tax deductions). Please note, there will not be an additional hourly wage or salary.
  • Receive a Segal Education Award upon successful completion of the term. The Education Award amount for this position is $7,395.00. Please note, this education award is an electronic transfer to either your educational institution, federal loan provider, etc. Members will not receive the education award via direct payment (such as check, cash, etc.).
  • This position is eligible for AmeriCorps childcare benefits (if member is income eligible).
