The Resource Specialist plays a pivotal role in facilitating clients' access to vital service entitlements and delivering support services. Specifically tailored for adults facing challenges related to mental health, substance use, and justice involvement, the Resource Specialist provides recovery-oriented services, including assessments, case management, skills training, and crisis intervention.Responsibilities:Assist with client intakes, collaborating with discharging facilities and other community agencies to ensure a seamless transition.Conduct field visits, including providing transportation for clients to and from essential services.Facilitate the linkage of clients to community services, actively participating in meetings and networking activities as required.Support the transition of clients to outside facilities, ensuring continuity of care and support.Conduct comprehensive assessments, with a focus on identifying needs related to benefits and housing support.Collaborate closely with the treatment team and support individuals to gather relevant information regarding clients' mental health and social needs.Assist clients in applying for and obtaining relevant benefits, such as General Assistance, Medicaid, Social Security Disability, and other financial assistance programs.Work collaboratively to develop comprehensive housing plans, considering safety, accessibility, and individual client requirements.Aid clients in navigating housing applications, subsidies, and support services to secure safe and stable housing.Identify appropriate housing resources, evaluating potential options to ensure they meet livability standards and align with clients' well-being.Advocate on behalf of clients to ensure fair and just treatment in securing benefits and housing.Collaborate closely with external agencies, social services, and community organizations to enhance available resources.Qualifications:High school diploma or equivalent (required)A minimum of 2 years of experience providing housing services to underserved populationsFamiliarity with fair housing laws and regulations, HMIS, SOAR (Preferred)Direct care experience in a mental health-related field providing services to individuals with a Co-Occurring diagnosis and/or involvement in the justice system (Preferred)Bilingual in Spanish , requiredFull Time | Monday to Friday | Onsite | 8am -5pm | Sacramento, CA 95811Pay Details: $20.00 to $24.00 per hourSearch managed by: Joanna CalderonBenefit offerings available for our associates include medical, dental, vision, life insurance, short-term disability, additional voluntary benefits, EAP program, commuter benefits and a 401K plan.
