United Health Services is Hiring a Nurse Care Manager Near Johnson, NY

Nurse Care Manager, UHS Binghamton General Hospital United Health Services is seeking a Nurse Care Manager to join our WMC Care Management department at UHS Binghamton General Hospital in Binghamton, NY. This position is benefits eligible.

$35.27 - $53.94 per hour, depending on experience.
Overview
Navigates and coordinates the care of patients across the continuum of care. Monitors and manages resources to transition patients to the appropriate level of care in a cost-effective fashion while ensuring quality outcomes and patient satisfaction. Assesses, plans, coordinates, monitors and evaluates the patients’ plan of care in collaboration with the healthcare team. Monitors the patient care process and collects data to evaluate and improve care and collaborates with physicians and other health care workers to provide comprehensive care including health risk appraisal, health promotion and clinical outcome studies. Educator payers, providers and the public and serves as the information source regarding state and national health care trends; initiatives, which enhance the provision of, care and ensure efficient resource utilization.

Shift: 7 p.m.- 7 a.m.
Qualifications
Education/Experience
Minimum Required:
  • Associate’s in Applied Science in Nursing
  • Minimum of two (2) years of experience in acute/tertiary facility
Preferred:
  • Bachelor’s of Science in Nursing
  • Five (5) years of experience in acute/tertiary facility
License/Certification
Minimum Required:
  • NYS Registered Nurse
