Clinical Manager Operations Area Director

POSITION SUMMARY:
The Home Health Care Clinical Manager is responsible for managing and supervising the clinical aspects of patient care at UEW. This includes overseeing the care provided to patients in their homes, ensuring compliance with all relevant regulations and standards, and leading a team of healthcare professionals.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Coordinate the entire care team to ensure proper screenings are performed to optimize patient care
  2. Follow hospitalized patients, obtain medical records, coordinate increase in hours as patient condition indicates
  3. Follow intake patients pending approval and FTF, perform weekly touchpoints to keep patients engaged
  4. Complete client processes including Plan of Care development, LMN, 60 Day Summaries, and Case Manager Summaries
  5. Ensure case managers are assigned and FTF visits are coordinated
  6. Serve as liaison between case managers and recruiting to ensure requisitions are active for open needs and assist with nurse interviews
  7. Onboard contract RNs and orient to EMR
  8. Oversee contract staff schedules, ensuring schedules are obtained and entered into the EMR
  9. Orient and mentor new case managers
  10. Lead weekly case conference
  11. Coordinate with the Retention Specialist for at-risk clients
  12. Coordinate with care team to ensure delivery of MD prescribed care
  13. Perform client visits, as needed
  14. Assist with required clinical and regulatory requirements (QAPI, JAR, PAG)
  15. Maintain compliance with HIPAA and applicable Health Care Privacy Law
  16. Supervise LPNs and HHAs per state and local regulations
  17. Ensure that the agency complies with all federal, state, and local regulations and accreditation standards.
  18. Generate reports and data analysis to track clinical performance and outcomes.
  19. Stay current with advancements in home health care practices and technologies.
  20. Facilitate ongoing education and training for clinical staff.
  21. Perform other duties as assigned

OTHER DUTIES AND RESPONSIBILITIES:

This position requires traveling between locations in your personal vehicle requiring a valid driver’s license and reliable transportation.

SUPERVISORY RESPONSIBILITY:

This position supervises LPNs and HHAs

Qualifications


PREFERRED QUALIFICATIONS:

  1. Current registered nursing license in good standing
  2. Current CPR certification
  3. Previous experience in home health care or a related healthcare management role.
  4. Knowledge of home health care regulations and accreditation standards.
  5. Experience in supervising and developing staff
  6. Experience utilizing an EMR
  7. Experience in an acute/chronic medical field with proven assessment skills
  8. Excellent communication and interpersonal skills
  9. Strong organizational and time-management skills
  10. Proficiency in Microsoft Office Suite
  11. Requires effective analytical and problem-solving skills
  12. May require some night and weekend hours to address issues that arise
  13. Must pass a criminal background check and drug screen
  14. Professional appearance required

WORK ENVIRONMENT and PHYSICAL REQUIREMENTS:

  1. Ability to sit at a computer terminal for an extended period of time.
  2. While performing the duties of this job, the employee is regularly required to, stand, sit; talk, hear, and use hands and fingers to operate a computer and telephone, keyboard, reach, stoop and kneel.
