United Energy Workers Healthcare is Hiring a RN Clinical Manager Near Grants, NM

  • Coordinate the entire care team to ensure proper screenings are performed to optimize patient care
  • Follow hospitalized patients, obtain medical records, coordinate increase in hours as patient condition indicates
  • Follow intake patients pending approval and FTF, perform weekly touchpoints to keep patients engaged
  • Complete client processes including Plan of Care development, LMN, 60 Day Summaries, and Case Manager Summaries
  • Ensure case managers are assigned and FTF visits are coordinated
  • Serve as liaison between case managers and recruiting to ensure requisitions are active for open needs and assist with nurse interviews
  • Onboard contract RNs and orient to EMR
  • Oversee contract staff schedules, ensuring schedules are obtained and entered into the EMR
  • Orient and mentor new case managers
  • Lead weekly case conference
  • Coordinate with the Retention Specialist for at-risk clients
  • Coordinate with care team to ensure delivery of MD prescribed care
  • Perform client visits, as needed
  • Assist with required clinical and regulatory requirements (QAPI, JAR, PAG)
  • Maintain compliance with HIPAA and applicable Health Care Privacy Law
  • Supervise LPNs and HHAs per state and local regulations
  • Ensure that the agency complies with all federal, state, and local regulations and accreditation standards.
  • Generate reports and data analysis to track clinical performance and outcomes.
  • Stay current with advancements in home health care practices and technologies.
  • Facilitate ongoing education and training for clinical staff.
  • Perform other duties as assigned
  • Current registered nursing license in good standing
  • Current CPR certification
  • Previous experience in home health care or a related healthcare management role.
  • Knowledge of home health care regulations and accreditation standards.
  • Experience in supervising and developing staff
  • Experience utilizing an EMR
  • Experience in an acute/chronic medical field with proven assessment skills
  • Excellent communication and interpersonal skills
  • Strong organizational and time-management skills
  • Proficiency in Microsoft Office Suite
  • Requires effective analytical and problem-solving skills
  • May require some night and weekend hours to address issues that arise
  • Must pass a criminal background check and drug screen
  • Professional appearance required
