Annual Salary Range $49,004.80 - $54,433.60

• Graduate of an accredited Practical Nursing program
• Active and in good standing Practical Nursing Licensure in State of Vermont
• Typing skills; Microsoft Office skills; electronic health record systems; e-mail use and etiquette
• Valid Driver’s License

Requirements

Under the direction and supervision of the Nurse Manager, the LPN organizes, plans, and supervises the medical component for assigned programs and individuals, satisfying State regulations which ensure the general health and well-being of agency clients.

MAJOR RESPONSIBILITIES:

• Report to Nurse Manager; collaborate with nursing team; assist prescribers as member of PMT.
• Complete visits to agency group homes; complete audits of Medication Administration Records, medication supplies; contribute to care planning and training as needed.
• Work with Group Home Managers on medical management, contacting PCPs, follow-up on order requests and refills.
• Complete home visits in collaboration with agency Case Managers as needed.
• Participate in chart audits and review medical paperwork, update EMR as needed.
• Identify client care needs, contribute to assessments, and report to Nurse Manager.
• Monitor medication refill line; stage medication prescriptions; monitor office medical supplies and medication samples as assigned.
• Administer IM injections in an outpatient setting weekly in collaboration with nursing team.
• Participate in Employee Health program.
