Job Summary
The Patient Navigator is accountable for facilitating patient continuity of care with the healthcare team. Acts in an intake model, assessing the patient needs, assisting with planning, organizing, implementing and evaluating the patient care needs with the healthcare team. Functions in direct support of the Hospitalist team and in partnership with other members of the healthcare team as a patient advocate, educator and coordinator.
Minimum Job Requirements:
Three years of experience as a Registered Nurse required. Previous case management experience or related experience preferred.
Current TX/Compact RN License required.
Current ACLS and PALS required.
Bachelor’s Degree in Nursing required.
