The BSAS School-based Program provides substance use disorder prevention services to students in their learning environment in public and private schools within Central Massachusetts. We offer assessment, social skills training, mentoring, family support, recreational activities, case management and consultation, and connection to community resources. Community Healthlink staff uses a nurturing and disciplined approach to help at-risk youth and teens deal with the issues that can distract them and prevent them from succeeding in the classroom. Benefits:Compensation based on education and related work experience, licensure, and other qualificationsHealth and Dental insurance (available upon starting)Generous paid sick, personal, holiday, and vacation timeCEU benefitsLife and Disability insurance401k with agency contributionWellness activity discountsWork related travel reimbursementResponsibilities: •Develop, implement, and review and Individual Service Plan (ISP) to meet the identified needs of each youth in an effort to effectively and appropriately coordinate service delivery.•Provide regular mentoring sessions that include teaching, coaching, and supporting development of a range of skills in youth.•Maintain client records (including completing Intervention plans, progress notes, and discharge summaries).•Communicate with collaterals for the purpose of treatment planning.•Participate in case consultation meetings.•Conduct psycho-educational groups with youth and family members.•Ability to assess basic physical, social, educational, legal, and service needs of client and coordinate care with other inter-agency and community-based providers.•Attends staff supervision, meetings and trainings as required.•Follow CHL protocols.•Some evening and weekends may be required.•Travel is required as part of this position.•Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors
