UltaCare Medical is seeking a Nurse Practitioner or Physician Assistant for a primary care position at a skilled nursing and long-term care senior living facility in Olney, IL. In this role, you will be the primary onsite advanced level provider responsible for the comprehensive care of the resident population. This includes addressing acute issues with urgency but also taking a pro-active approach in treating the full spectrum of chronic conditions.
The ideal candidate for this position is a compassionate and proficient healthcare professional who prioritizes patient care above all. You'll independently assess residents' medical needs and manage your schedule to ensure timely care. While autonomy is key, you'll also collaborate with the off-site Medical Director and other specialized providers, balancing independent decision-making with integrating with an interdisciplinary team.

Key Responsibilities:
  • Conduct medical evaluations, treatments, and consultations for facility residents.
  • Proactively manage chronic conditions to minimize disease progression, lessen the risk of major acute events, alleviate side effects, and enhance quality of life for residents.
  • Address and manage acute medical situations as they arise with urgency and compassion.
  • Collaborate effectively with facility staff and the Medical Director.
  • Issue medical orders for prescriptions, medication administration, tests, and referrals.
  • Communicate with residents' families when necessary.
  • Promptly document patient interactions within 24 hours using dictation.
  • Review test results and ensure patient notification.
  • Deliver patient education as required.
Qualifications and Experience:
  • Must hold an active Advanced Level Practitioner license, open to both Nurse Practitioners (NP) and Physician Assistants (PA).
  • Capability to independently identify patients for care, set personal schedules, and establish effective relationships with facility staff.
  • Proficiency in working autonomously and making independent decisions.
  • Experience in an institutional inpatient setting is preferred.
  • Prior work in skilled nursing and/or long-term care facilities is advantageous.
  • Experience with geriatric patients is desirable.
  • Experience in other settings including traditional outpatient clinics, urgent care clinics, and other specialty clinics will also be considered. 
Job Types: Full-time, Part-Time, Employee or Independent Contractor.
Salary: $105,000-$110,000 with uncapped monthly productivity bonus. (average range is additional $500-$3000 per month)
