UHS of Delaware, Inc. is Hiring a HOUSE SUPV (RN) Near MADERA, CA

Responsibilities

River Vista Behavioral Health sits along the bluffs overlooking the San Joaquin River, this brand new, 128-bed facility, will provide high-quality and advanced behavioral health services to residents and visitors in the Central Valley. The new hospital will employ more than 250 people, including clinicians, nurses, mental health technicians, support staff and administration.

We are currently seeking a forward thinking and talented RN, House Supervisor to be part of our Leadership Team. The House Supervisor (RN) is responsible for ensuring coordinated delivery of patient care in accordance with the established standards of care, mental health practice and hospital policy, goals and objectives. This position assists in contributing to the hospital's philosophy, objectives, educational and performance improvement efforts, while providing supervision to the nursing staff. The House Supervisor is responsible for direct supervision of the nursing staff, patients, and patient care during their shift. The House Supervisor will work closely with the Intake and Admissions Department regarding patient admissions, discharges, and transfers during their shift.

QUALIFICATIONS

Education: Graduate from an accredited school of professional nursing required or equivalent combination of education and experience in psychiatric nursing management; Bachelor's Degree preferred.

Experience: A minimum of three (3) years’ experience in a psychiatric health-care facility preferred.

Licensure: Currently licensed to practice by the California State Board of Nursing.

Additional Requirements: Thorough knowledge of The Joint Commission, OSHA requirements, and state and federal regulatory requirements. CPR certification and successful completion of training in CPI/HWC within 30 days of employment, and prior to assisting in restraining procedures.

STANDARDS OF PERFORMANCE

THE ESSENTIAL JOB FUNCTIONS ARE CATEGORIZED UNDER THE JCAHO STANDARDS OF PERFORMANCE.

Leadership

  • Ensure that all staff members deliver care in accordance with the Standards of Care and Practice.
  • Monitor that staff are implementing policies and procedures and act as a resource to the staff.
  • Responsible for unit compliance with JCAHO, HCFA, state and regulatory standards and patient rights standards. 
  • Evaluate program effectiveness and help formulate plans for improvement. 
  • Assist in developing staffing schedules in accordance with patient acuity And staffing grid.
  • Coordinate the process for determining and maintaining accurate legal processes for patients.
  • Ensure Infection Control, Safety and Risk Management policies are being followed by staff.
  • Coordinate and evaluate the effectiveness of treatment planning offered by Nursing
  • Process unusual occurrence reports, conduct investigations and Corrective plans when needed.
  • Participate in peer review committee and performance improvement activities.
  • Monitor medical records for appropriate documentation of care and effectiveness.
  • Collaborate with therapists, physicians, Charge nurses to ensure patients receive quality care.
  • Collaborate with the Care Center on admissions, discharges, and transfers.
  •  Personnel Management

  • Assist with recruitment and interviewing potential staff.
  • Assist in ensuring all program staff is evaluated according to personnel policies.
  • Assist in providing supervision and direction in accordance with Nursing Department's goals and objectives to all nursing staff and conducts disciplinary actions where indicated.
  • Continually evaluate the activities, morale and interpersonal relationships of the program staff and promote methods to improve these areas.
  • Assist in ensuring that all new employees are oriented to the program.
  • Assist in providing education, orientation and in-services for staff when deficits in clinical knowledge are identified.
  • Treatment Planning

  • Monitor nurses for development and implementation of the nursing care plans.
  • Verifies the patient's care through direct observation and consistent review of the chart.
  • Teaching

  • Provide education to patients and families when needed.
  • Answer patient and families questions about the hospital and treatment.
  • Monitor that staff are providing appropriate teaching to patients.
  • Milieu

  • Monitor that staff are adequately orienting patients to the unit.
  •  Monitor that staff are engaging patient’s participation in activities.
  • Consistently utilize universal precautions and environmental safety guidelines.
  • Utilize self as a role model to promote health communication.
  • Intervention

  • Employ principles of communication, interviewing techniques, problem solving, and crisisintervention with patients and families.
  • Transcribe physician orders accurately if orders are received.
  • Use appropriate interventions in psychiatric and medical emergencies.
  • Respond to all codes and assist staff as needed.
  • Review all paperwork and ensure that physician response to a psychiatric emergency is timely.
  • Assessment

  • Complete a comprehensive nursing assessment, patient interview and medication reconciliation when needed in the Care Center.
  • Assess the units and determines staffing needs for next shift and report to ACNO.
  • Report any signs or symptoms of infection in patients to the Infection Control Nurse.
  • Verify the accuracy of patient records including patient medication reconciliation.
  • Ensure that nurses complete a nursing assessment within 8 hours of patient admission.
  • Ensure that IM consults and H&Ps are logged and completed by the IM physician.
  • Specific Standards for the Care of Child Patients Ages 5 through 11

  • Maintain knowledge of the Child Program rules, protocols and handbooks guidelines.
  • Maintain knowledge of children’s stages of development and seek guidance from the nurse in charge when questions about the norms for behavior related to child development arise.
  • Assess patients for behaviors consistent with the developmental stages and make note of any deviations in behavior.
  • Recognize acting out behavior of child specific age group and set appropriate limits in clear, concise, behavioral and non-judgmental terms based on the age of the patient.
  • Demonstrate an awareness of learning disabilities and translates treatment/program expectations to the learning disabled child.
  • Relate to child patients in an age appropriate manner.
  • Utilize seclusion and restraint procedures with an understanding of the need for modification in regard to smaller body sizes of children, abandonment issues and the history of physical and/or sexual abuse issues.
  • Act as a resource to other staff in regard to the Child Patient Programs.
  • Specific Standards for the Care of Adolescent Patients Ages 12 through 18

  • Maintain knowledge of the Adolescent Program rules, protocols and handbooks guidelines.
  • Assess patients for behaviors consistent with the developmental stages and makes note of any deviations in behavior.
  • Recognize acting out behavior of adolescent specific age group and set appropriate limits in clear, concise, behavioral and non-judgmental terms based on the age of the patient.
  • Demonstrate an awareness of learning disabilities and translates treatment/program expectations to the learning disabled adolescent.
  • Relate to adolescent patients in an age appropriate manner.
  • Utilize seclusion and restraint procedures with an understanding of the need for modification in regard to smaller body sizes of adolescents, abandonment issues and the history of physical and/or sexual abuse issues.
  • Act as a resource to other staff in regard to the Adolescent Patient Programs.
  • Specific Standards for the Care of Adult Patients Ages 19 through 64

  • Gather information from the patient, family and interdisciplinary team and use judgements based on knowledge of adult behaviors in treatment planning and interventions.
  • Recognize behaviors and set appropriate limits in clear, concise, behaviors and non-judgmental terms.
  • Utilize seclusion/restraint procedures applicable to adult body size and past history of violent or emotional behaviors and/or history of physical and/or sexual abuse issues.
  • Maintain current knowledge of the Adult Program rules, protocols and handbook guidelines.
  • Utilize theoretical concepts to guide the effective practice of adult care within Specific Standards of Care for this age group.
  • Specific Standards for the Care of Geriatric Patients Ages 65 and older

  • Maintain knowledge of the developmental tasks and sociological theories of aging.
  • Utilize theoretical concepts to guide the effective practice of gerontological health care and behavioral care.
  • Gather the appropriate information from the patient, family and interdisciplinary team and use therapeutic judgements based on knowledge of gerontological health/behavioral care practices to develop a comprehensive plan of care.
  • Utilize interventions based on gerontological theory to restore patients’ functional capabilities and to prevent complications and excess disability.
  • Maintain a complete knowledge of treatments, medications and side effects of such in the geriatric age group.
  • Specific Standards for the Care of Chemically Dependent Patients

  • Assesses patient needs as related to chemical dependency and document the plan of care from the admission through discharge.
  • Collaborate with other chemical dependency staff professionals in planning for discharge and follow-up care.
  • Maintain an awareness of how personal issues relating to co-dependency can impact work performance/relationships.
  • Recognize and establish limits on medication-seeking behaviors.
  • Performance

  • Provide supervision to the nursing staff and acts as a resource for hospital policies and procedures.
  • Attend annual in-service, mandatory educational programs, and participate in PI initiatives.
  • Comply with established measures for infection control, OSHA regulations and risk management.
  • Use time constructively and in an organized manner to accomplish assigned responsibilities, delegates duties to other personnel as appropriate, and demonstrates good judgment ability.
  • Act as the patient's advocate and assure that patient rights are upheld.
  • Demonstrate guest relations in communication, cooperation and in giving assistance to patients, physicians/licensed practitioners, coworkers and visitors in a prompt and courteous manner.
  • Communicate with Administrator on Call, CNO, ACNO, Program Directors, and Director of Clinical Services on all incidents that occur on the shift.

  • Ability to sit and stand (one-third of the shift) and walk (two-thirds of the shift).

  • Ability to work in an environment where chemicals are frequently used for cleaning, where mechanical and electrical hazards may occasionally be present and where dust, mist and steam are frequently generated in housekeeping tasks.
