Responsibilities

The UHS Southern CA Region consists of five acute care facilities located in the Southern CA area; 

  • Temecula Valley Hosptial in Temecula, CA
  • Southwest Healthcare System: Inland Valley Medical Center in Wildomar, CA and Rancho Springs Medical Center in Murrieta, CA
  • Corona Regional Medical Center in Corona, CA
  • Palmdale Regional Medical Center in Palmdale, CA

POSITION SUMMARY:

The Health Information Management (HIM)/Medical Records Analyst collaborates with staff across the Region and is responsible for auditing medical record charts and accurately reviewing them according to established Medical Staff Bylaws, Medical Staff Rules and Regulations and Federal and State Statutes.

POSITION SPECIFIC RESPONSIBILITIES:

  • Consistently demonstrates the ability to conduct quality analysis on inpatient, observations, ASC, Emergency Room and Outpatient services within 48-hours of discharge/visit according to the current medical staff bylaws, rules and regulations, and State and Federal statutes and The Joint Commission Standards.
  • Consistently accounts for 100% of all charts analyzed on the discharge lists within 24-hours of discharge on the first working day.
  • Accurately applies appropriate deficiency within the electronic record system with 95% accuracy or greater.
  • Assists providers in completion of their records and to answer questions regarding completion requirements.
  • Conducts accurate concurrent reviews according to the current medical staff bylaws, rules and regulations, and State and Federal statutes and The Joint Commission Standards.
  • Compiles accurate statistical data on chart audits concurrently and/or after discharge for reporting purposes using the latest performance improvement tools/methods.
  • Follow-up with each provider with a phone call/meeting when an electronic communication is ineffective to ensure the provider has an understanding of a process/procedure to meet compliance.
  • Prepping Medical Record requirements:
    • Knowledge of all medical record forms.
    • Knowledge of barcodes in association with medical record forms.
    • Ability to research and correct account and medical record numbers on documents.
    • Accurately prepare all documents in correct order to minimize sorting errors with 95% accuracy in eMR.
  • Scanning requirements:
    • Daily maintain, clean and troubleshoot all scanning equipment.
    • Use proper protocol to report any mechanical issues to Information Services (I.S.) department at all times.
    • Use proper protocol to report any technical issues related to scanning for all issues.
    • Accurately select job type for all scanning batches 100% of the time.
    • Perform quality control of the image in Scan Station to ensure quality and quantity of documents scanned 95% of the time.
  • Promotes effective working relationships and works effectively as part of the team to meet departmental goals and objectives.Consistently demonstrates the ability to assist in answering phones and responding appropriately to requests. To promote service excellence, phones shall be answered by the third ring.
  • Participates in monthly meetings or reviews monthly minutes and applies information and any changes to areas of responsibility.
  • Communicates any hand-off information for your primary and secondary function(s) to the responsible staff member to ensure continuous workflow when you have knowledge of upcoming absences.
  • Maintain an accuracy rate of 95% or greater for all work analyzed.
  • Perform quality control functions on all objects scanned comparing the paper record to the scanning system ensuring quantitative and qualitative documentation at a 95% accuracy rate.

  • Performs corrections within the scanning system utilizing merge, split, re-order, rotation-save and scanning functions at a 95% accuracy rate.
  • Track prepping and scanning errors discovered through the quality control process to help with performance improvement.
  • Performs corrections within the current EHR system or notify appropriate staff member for correction when discovered at a 95% accuracy rate or greater.
  • Meet all productivity standards set forth by UHS maintaining an average rate of 95% or greater for analaysis and 95% for prepping and scanning processes.
  • Daily document all productivity data within your individual spreadsheets.
  • Performs other duties as assigned.

Qualifications

EDUCATION/TRAINING/ EXPERIENCE:

  • High School Graduate or equivilent required.
  • Associate’s degree from an accredited College or University in Medical Records Science preferred.
  • Completion of an introductory course of Medical Terminology required.
  • Two (2) to Three (3) years of experience in Medical Records required, acute care experience preferred.
  • Prior knowledge of State and Federal release of information laws preferred.

CERTIFICATIONS/LICENSES:

  • Must have valid California Driver’s License and reliable transportation.
