PRN Discharge Planner Opportunity

Our Location:

We are located in picturesque Marion County, FL. The county is known as the Horse Capital of the World. Enjoy the outdoors at one of the county’s serene parks, which includes wonderful hiking trails, river rafting, and tubing. Watch the county’s wildlife and view the scenery by taking a horseback ride through one of the area’s riding trails. Considered the heart of Florida, the beautiful region is centrally located within hours of many activities. Just a drive away from some of the state’s amusement parks or beautiful Florida beaches. The county is less than two hours away from some of the state’s large metropolitan areas. From young professionals to a growing family, the area has plenty of activities for every individual.

We currently are looking for a PRN Discharge Planner to join our team of dedicated Behavioral Health professionals working with adults dealing with psychiatric and/or substance abuse issues. Therapists conduct therapeutic and case management services for adult patients. They provide individual, family and group therapy. Therapists are responsible for developing the psycho-social history, assessing the therapeutic needs of the individual and family and developing appropriate treatment plans. The Therapist assumes responsibility for the overall discharge planning needs and placement of patients. Therapists act as the patients and families' primary contact during the patient's stay at the facility as well as their advocate with the treatment team.

Hours: varies per week between the hours of 8am-4:30pm

Qualifications

Requirements:

• Bachelor’s Degree from an accredited college or university in Social Work, Mental Health Counseling or other related fields.
• Two (2) years direct clinical case management experience specific to family, individual social service issues and community services for next lower level of care as it relates to discharge planning processes.
• Must have work related experience and references available upon request; License or Certification a plus.
