Under the direction of the Nursing Services team, the Certified Nurse Assistant provides direct patient care, aids with activities of daily living, and assists in the maintenance of a safe and clean environment. Responsible for performing duties in accordance with hospital policies and procedures in collaboration with the nursing services team. Duties include but not limited to:

SAFETY AND ASSESSMENT

  • Ensure safety of patients at all times. Consistently provide supervision for assigned patients.
  • Assess patients on an ongoing basis for any signs or symptoms of distress or physical/mental deterioration and report promptly to the Charge Nurse. Identify and implement measures, in collaboration with licensed nursing staff, to increase patient comfort and safety.
  • Consistently record and/or report to the Milieu Specialist or Charge Nurse any treatments, results, observations, patient complaints, and other pertinent information regarding the condition of the patient.
  • Recognize changes in vital signs, LOC, breathing patterns, diet, bowel or bladder habits, skin condition, or hygiene. Report changes to Charge/Staff Nurse in a timely manner.
  • PATIENT CARE / SKILLS / TASKS

  • Review treatment plans and follows plan of care, which may include special care or monitoring.
  • Perform daily patient care tasks, including feeding and bathing patients. Performs basic housekeeping tasks required to maintain patient living areas, which may include cleaning soiled beds and floors.
  • Obtain accurate vital signs, height and weight, intake and output, and documents information appropriately.
  • Assist with admission, transfer, and discharge processes under the guidance of the Charge/Staff Nurse.
  • Escort patients to events away from the facility such as Court for conservatory or other hearings, Emergency Room or clinician office as necessitated by their physical condition.
  • Provide care to patients with medical problems such as: hypertension, diabetes, URI, UTI, heart disease, thyroid disorders, and/or acute or chronic infection. Notify Charge/Staff Nurse of signs of distress. Provides for patient’s comfort and activity according to patient’s tolerance level.
  • MILIEU MANAGEMENT

  • Prioritize interventions in rapidly changing patient situations. Use observational skills and provide feedback to patients.
  • Utilize least restrictive and most facilitative measures to de-escalate patients.
  • Assist in providing intervention and/or management of inappropriate behavior in the milieu. Available for 1:1 interaction with patients. Demonstrate skills in co-leading groups and activities.
  • Provide care for patients with: depression/suicidal impulses, mania, delusions/hallucinations, paranoia, anxiety, aggressive/assaultive behavior, chemical dependency issues/detox, personality disorders, and/or dementia/Alzheimer’s.
  • ROUNDS

  • Complete rounds and document findings accurately and timely, in accordance with hospital policy and procedures.
  • Follow appropriate observation level for patient: every 15 minutes or 1:1. Utilizes and monitors for specific precautions as ordered.
  • Recognize changes in patient’s affect, behavior, mood and level of suicidality or self-harm. Report changes or issues of concern to the Charge Nurse immediately.

    Qualifications

    MINIMUM REQUIREMENTS OF THE POSITON:

    • High School Diploma or Equivalent required
    • Current Certified Nursing Assistant certification with the California Department of Public Health required
    • Current CPR certification required
    • Experience in a psychiatric setting preferred.
    • Must be able to work a minimum of one (1) weekend shift per week

    Knowledge/Skills:

    • Must be able to interact positively with patients whose behavior may be difficult, uncooperative or aggressive;
    • Must be capable of adapting to varying workloads and work assignments on a constant basis;
    • Must be able to perform concentrated and/or complex mental activity with frequent involvement in complex and/or highly technical situations;
    • Must be able to work successfully under highly stressful conditions;
    • Must be able to make sound, independent judgments based on scientific and/or ethical principles;
    • Must be able to comprehend and perform oral and written instructions and procedures;
    • Must be able to collaborate with other multidisciplinary team members in an appropriate fashion;
    • Must have effective comprehensive reading skills, strong communication skills, written and verbal.
    • Must possess a valid California Driver’s License in order to transport patients and/or staff, or drive hospital vehicles.
