Wellstone Regional Hospital is a 100-bed acute care facility located in Jeffersonville, Indiana and has been providing quality health care to the residents of Southern Indiana and the Louisville area since 2003. Wellstone specializes in the treatment of Adolescents, Children, Adults, and the CD population. In addition to our in-patient services, we offer out-patient programs as well.
Wellstone is currently seeking out a Case Manager/Discharge Planner in our Social Services Department. The Case Manager/Discharge Planner assists with social services functions including communication with referral sources at admission, throughout treatment, and at discharge; families, physicians, and schools, Case Managers will ensure patients/families are scheduled after care, and providing information and referrals as needed. They will assist with safety planning, community resources, discharge satisfaction surveys, and referral letters. Case Managers will maintain consistent communication with guardians and Department of Child Services to meet the treatment and discharge planning needs of the patient. Case Managers will assist social workers in filling needs, medical record copies, and returned phone calls with insurance case managers. Case Managers will work hand in hand with assigned social workers and a multidisciplinary team to meet the needs of the consumer as providing service excellence.
QUALIFICATIONS
Education: Bachelor’s Degree from an accredited college or university, in Social Work or a clinical related mental health field.
Experience: Two (2) years’ experience in a mental health care facility preferred; must have strong communication skills.
Additional Requirements: CPR certification and CPI training within 30 days of employment and prior to assisting in a restraining procedure.
Hours of Work: May be required to work flexible hours but primarily day shift, 40 hours per week. This is an hourly non-exempt position.
