Full time Case Manager for the Grants to Benefit Homeless Individuals program located in Batavia, Genesee County, NY.

Description of Duties and Responsibilities: The 1.0 FTE Grant Funded program. The Case Manager will assist individuals and provide support for emergency, transitional, and ultimately permanent housing obtainment for people with substance use disorder (SUD) who are homeless. He/she will be responsible for coordinating substance use disorder (SUD), mental health, and physical health care provider communication and navigating health insurance and safety net program enrollment and maintenance processes for people with SUD who are homeless, at imminent risk of homelessness, and/or have histories of chronic housing instability. Additionally, the Case Manager will provide or will provide access to various supportive services, including: linkages to community resources and educational and vocational programs; job counseling and job readiness training; support for ensuring the educational and developmental needs of children are met; finding and obtaining safe housing; access to Peer Recovery Advocates/Coaches; assistance identifying and securing safe, affordable childcare options; assistance connecting with legal resources; universal skill-building such as coping, communication, collaboration, problem-solving, action planning, goal setting and monitoring, resource management, etc.; among others. Support services offered will be individualized based on the needs of the individual in treatment or recovery.

Qualifications: Minimum of 1 year of case management or similar experience required. 2 or more years and/or intensive case management experience preferred. Bachelor’s degree preferred. Excellent communication skills. Ability to work with individuals and families who have substance use disorder and a variety of life challenges that result in a need for support in order for the individual/family to remain stably housed. Knowledge of the varying modalities and philosophies of alcohol and substance use disorder treatment and recovery, including a solid understanding of and willingness to support harm reduction approaches. Commitment to frequent face-to-face meetings with participants and additional check-ins as needed via other communication modes (e.g. phone, etc.). Knowledge of community resources and programs, including but not limited to safety net programs. Ability to effectively communicate and collaborate with representatives from other health care agencies and public and community agencies. Commitment to providing high-quality, complete participant notes.

Reporting Relationship: This position is under the supervision of the GBHI Grant Coordinator. Services and will need to be able to collaborate with Treatment and Residential program leaders.

Valid NYS Driver's license and method of transportation required. Must be free of active addiction, including tobacco. Pre-employment background check/post offer drug screening required. CARF and NHSC accredited. EOE

Pay: $17.00 - $20.00 per hour
