Will work _37.5_ hours, any hours over 40 will be considered overtime.
*Requirements:*
§ Ohio licensed Social Worker or Registered Nurse in good standing with the credentialing board.
§ At least one year of experience working with persons with disabilities/chronic conditions and LTSS.
* Prior case management experience and prior experience completing electronic documentation a plus
§ Ability to effectively use clinical judgment
§ Have above average technology skills
§ Knowledge of community resources within Southwestern Ohio.
§ Good customer service skills.
§ Good interpersonal communication skills.
§ Ability to adapt to frequent change.
* Ability to collaborate and problem solve in a self-directed team environment.
§ Good oral and written communication skills.
§ Good organizational and teamwork skills.
§ A valid driver’s license and reliable transportation for travel within Butler, Clermont, Clinton, Hamilton and Warren Counties.
§ Ability to travel within the state of Ohio for training.
§ Respect the cultural, spiritual, racial, ethnic beliefs of others.
*Physical Demands:*
§ Sitting for long periods of time while documenting, entering data into the computer, completing forms, responding to phone calls, and meeting with consumers.
§ Using hands and arms to write and enter data into the computer.
§ Stretching and bending to file or retrieve data.
§ Talking on the phone for extended periods and communicating with all customers.
§ Walking up stairs to enter consumers’ homes.
§ Driving to and from consumers’ homes and other community settings in the COA 5-county area or within the State of Ohio for meetings or training.
§ Visual observation skills are critical to job functions in assessing and evaluating consumer conditions.
§ Must have the physical mobility to make independent home visits.
§ Carry a laptop computer, folding stool, and assorted packets to and from visits in a variety of physical conditions or situation.
