The Health Information Management Department is comprised of seven different units with a staffing of over 180.45 FTEs whose primary purpose is to serve the interests of the individual patient by facilitating medical care, treatment, research and teaching through availability of well documented, current medical record and clinical data base. This position is chiefly responsible for ensuring the physician’s clinical documentation appropriately captures resource intensity and severity of illness. Responsible for concurrent review and analysis of the clinical documentation in the medical records using query of the Medical Staff and other care givers as necessary with documentation query forms or verbal communication to obtain accurate and complete clinical documentation that appropriately supports the severity of patient illness and the intensity of the services ordered. Also assists in education of Medical Staff and other care givers relating to clarity and completeness of the clinical documentation.

Minimum Qualifications

• Possession of current RN license; Graduation from an accredited school of nursing with a minimum of 3 years recent experience in an acute care setting.
• Ability to read/abstract handwritten and transcribed documents in the medical record, and interpret the clinical information.
• Comprehensive knowledge of medical diagnostic and procedural terminology required.
• College level understanding of disease processes, anatomy and physiology necessary to interpret critical lab values, medical necessity of diagnostic and therapeutic procedures, and the administration of medications.
• Knowledge of and understanding of multi-medical specialties, and procedures in acute and critical care setting.
• Broad-based clinical experience with a demonstrated experience in patient care delivery in critical and acute care setting.
• Ability to read and comprehend English text and medical terminology; knowledge of medical and nursing treatment plans, levels of care associated with acute and critical care.
• Skill to perform in-depth chart analysis to provide accurate information in a concise, informative manner, identify/analyze problems or issues related to areas of responsibility.
• Must be able to work occasional overtime.

Preferred Qualifications

• BSN preferred.
• UR or critical care nursing experience preferred.
• Knowledge of DRG reimbursement system preferred.
