Job Title: RN Case Manager - PACE/LIFE Program
Responsibilities:
- Provide direct patient care and case management services to seniors with chronic health conditions in the PACE/LIFE program.
- Conduct comprehensive evaluations and assessments to determine patient needs, focusing on long-term care and support in their own homes.
- Work collaboratively with interdisciplinary healthcare team members, including physicians, nurses, and other healthcare professionals, to coordinate and implement patient care plans.
- Monitor patient progress, adjust care plans as needed, and provide ongoing case management.
- Engage in effective discharge planning and ensure smooth transitions of care for patients.
- Apply case management principles to optimize patient outcomes and efficient resource utilization.
- Educate and support patients and their families regarding healthcare needs, chronic disease management, and lifestyle choices.
Experience:
- Must possess a current Florida Registered Nurse (RN) license.
- At least 2 years of nursing experience, with a preference for experience in geriatric care, hospice, home health, or a similar setting.
- Experience in case management, particularly within a PACE/LIFE program, or related fields such as discharge planning or utilization management, is highly desirable.
- Strong understanding of geriatric physiology, chronic disease processes, and treatment modalities.
- Excellent communication and interpersonal skills for effective collaboration with patients, families, and a diverse healthcare team.
- Demonstrated ability to manage multiple cases effectively while maintaining meticulous attention to detail and compassion.
