Responsibilities:

- Provide direct patient care under physician’s orders and RN supervision, focusing on patients in times of crisis to maximize comfort and enhance quality of life.

- Perform nursing assessments, including monitoring vital signs and response to treatments, and administer medications as prescribed.

- Collaborate with healthcare team members, including the RN case manager, to develop and implement individualized care plans.

- Provide continuous bedside clinical observations and report significant changes in patient condition and family needs.

- Offer supportive care and emotional support to patients and families, ensuring a compassionate and respectful environment.

- Document all clinical services in the patient’s medical record accurately and timely, according to PACE documentation requirements.

- Assist with complex cases as needed, utilizing strong clinical judgment and problem-solving skills.

Skills:

- Valid Florida LPN license and graduate of an approved nursing program.

- Minimum one year experience in an acute care setting, providing direct patient care; hospice experience preferred.

- Proficiency in electronic medical record systems.

- Strong communication skills for effective interaction with patients, families, and healthcare team members.

- Ability to manage emotional and challenging situations with sensitivity and professionalism.

- Committed to following infection control protocols and safety measures.

Additional Requirements:

- Willingness to work three 12-hour shifts, including weekends.

- Ability to travel across the designated coverage area.

- Participation in the seasonal influenza program as a condition of employment.
