We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to individuals with substance abuse disorder. Your role will involve assessing client needs, developing care plans, and connecting clients with appropriate resources and services. Responsibilities:
- Conduct comprehensive assessments of client needs and develop individualized care plans
- Provide ongoing case management services to clients, including regular check-ins and follow-ups
- Connect clients with community resources, such as housing assistance, job training programs, and healthcare services
- Collaborate with other professionals, such as social workers, counselors, and healthcare providers, to coordinate services for clients
- Utilize motivational interviewing techniques to empower clients and promote positive behavior change
- Assist clients in accessing government assistance programs, such as Section 8 housing or food stamps
- Provide crisis intervention and support during times of emotional distress or emergency situations
- Supervise and monitor client progress towards achieving their goals
- Maintain accurate and up-to-date case notes and documentation

Skills:
- Strong mentoring skills to guide clients towards self-sufficiency and personal growth
- Knowledge of workforce development programs and resources to assist clients in finding employment opportunities
- Familiarity with social work principles and practices to provide effective support to individuals and families
- Proficiency in motivational interviewing techniques to engage clients in the change process
- Experience in addiction counseling to address substance abuse issues among clients
- Ability to handle crisis situations calmly and effectively through crisis management techniques
- Excellent communication skills, both verbal and written, for effective collaboration with clients and other professionals
