POSITION SUMMARY:

Provides interpreting and case management services for Reproductive Health clients coordinate referrals to community agencies for additional assistance, treatment, and continuity of care.

PRINCIPAL DUTIES/RESPONSIBILITIES:

The following functions represent the majority of the duties performed by the position, but is not meant to be all-inclusive or prevent other duties from being assigned when necessary.

ESSENTIAL JOB FUNCTIONS (95% of Time):

1. Coordinate services with THD contract provider for clients receiving tubal ligation and vasectomies.
2. Coordinates patient care, patient contact, and referrals for family planning and adult health clients with nursing staff.
3. Participates in educational health sessions, health fairs and/or in-service training to increase positive health practices in the Latino community.
4. Assists clients with transportation needs.
5. Assists with scheduling clients for family planning and adult health services, in coordination with nursing staff. 
6. Coordinates existing clients’ care with Social Worker/Case Manager and Outreach staff.
7. Assists Outreach staff with Spanish reminder calls for Family Planning and Adult Health clients.
8. Assists with dysplasia history information and client education during colposcopy clinic. 
9. Assists staff with Spanish reminder calls and clients calls. 
10. Provide interpretation for staff when needed and provides English/Spanish translation of written materials. 
11. Assists in Family Planning and Adult Health Clinic with history completion information during staff shortage.
12. Assists with client referral, application and completion of SoonerCare enrollment.
13. Completes required training in support of duties and responsibilities of this position.
14. Other duties as assigned including those required to fulfill activities in support of public health emergency operations.

SECONDARY JOB FUNCTIONS (5% of Time):

1. Participates in staff meetings, quality assurance activities and in-service training.

2. Completes monthly time sheet and mileage forms.

QUALIFICATIONS/SPECIFICATIONS:

Education: High school diploma or equivalent required. Spanish translator certificate a plus.

Experience: Experience in translating/interpreting in health care preferred.One year experience in public health or health care preferred. 

Knowledge and Skills:

• Excellent interpersonal and good communication skills are required to provide effective client counseling and obtain cooperation from clients who come from a wide range of economic, social, and ethnic backgrounds.
• Skills in interviewing and proving health education; and in working with agency staff and community partners.
• Skills in obtaining and protecting personal health information (PHI).
• Effective interpersonal skills in working with personnel at all organizational levels.
• Bilingual English/Spanish required.

Licenses/Certification:

• Valid Oklahoma driver’s license.
• Certification in Spanish language interpretation preferred.

INTERNAL AND EXTERNAL WORKING RELATIONSHIPS:

• Regular internal contact with various nurses to coordinate care or referrals for patients.
• External contact with community partners for the coordination of care and referrals for clients.

• Must maintain THD record confidentiality according to HIPAA regulations.
• Must possess ability and willingness to perform job-related travel
