Administrator experience is a plus for this DON role!!!

Job Summary:

Hospice RN provides constant and ongoing care for elderly and terminally ill patients by evaluating patient needs, creating care plans, and providing end-of-life support to patients and their families. This position may require flexible shifts that include nights, weekends, and holidays. Th work in collaborative, team-driven environments that require frequent communication with other caregivers. As a Hospice Registered Nurse for Homestead Hospice, you have the special opportunity to provide one on one hospice and palliative care to terminally ill patients in their home. 

Qualifications:

1. A current nursing License in the state, with at least 3 years of experience in nursing with one-year in-home health/hospice.
2. Familiarity with a variety of the clinical concepts, practices, and procedures.
3. Ability to exchange routine information in an appropriate manner.
4. Good oral and written communication skills.
5. Hospice experience preferred 

Responsibilities:

1. Responsible for assigned visit schedule, meeting the approved visit frequency.
2. Maintains a nursing service philosophy and related rules, standards, and practices; establishes care plans, including assessments, plans of treatment, objectives and goals, evaluations, and discharge planning, maintains current medication profiles, and timely re-order of hospice appropriate medications.
3. Ensures compliance with laws and rules related to care, certification, and licensing; professionally represents Creative Hospice Care to residents, residents’ families, outside agencies, care providers, and the public.
4. Maintains liaison role with all levels of administration, physicians, and outside organizations.
5. Complete required documentation in the time frame allotted—nurses' notes turned in at least 2 times per week, admission packets turned in within 24 hours of the admission, discharges turned in within 24 hours of the discharge, recertification packets turned in by the due date of recent.
